目的探讨乳腺癌患者的超声征象与组织中色素上皮衍生因子(PEDF)蛋白表达的相关性。方法选择2013年1月至2015年6月间宝鸡市妇幼保健院收治的298例乳腺癌患者为研究组,采用随机数字表法随机选取同期门诊收治的200例乳腺导管上皮增生性疾病患者为对照组,分析两组患者肿块的超声征象和血流情况,探讨不同超声征象和不同血流分级患者中PEDF蛋白的表达情况。结果研究组患者乳腺癌组织中,PEDF蛋白的阳性表达率为28.4%,显著低于对照组的65.5%,差异有统计学意义(P<0.05)。彩色多普勒超声血流0级、Ⅰ级、Ⅱ级和Ⅲ级的PEDF蛋白阳性表达率分别为50.5%、38.8%、29.6%和10.9%,随着血流分级的升高,PEDF蛋白的阳性表达率越低,差异有统计学意义(P<0.05)。肿瘤直径≤2.0 cm患者的PEDF蛋白阳性表达率为48.2%,显著高于肿瘤直径>2.0 cm的10.4%,差异有统计学意义(P<0.05)。不同形态、边界、后方回声、纵横比、内部钙化以及乳腺影像报告和数据系统(BI-RADS)分级患者的PEDF蛋白阳性表达率差异均无统计学意义(均P>0.05)。结论乳腺癌超声征象可在一定程度上反映肿瘤组织中PEDF蛋白的表达水平,对乳腺癌患者预后的评估具有重要的指导意义。
Objective To investigate the correlation between ultrasound signs and the expression of pigment epithelium-derived factor (PEDF) protein in breast cancer patients. Methods A total of 298 breast cancer patients admitted to Baoji MCH from January 2013 to June 2015 were selected as the research group and 200 patients with proliferative breast ductal epithelial disease Group, analyze the ultrasound signs and blood flow of the two groups of patients, and discuss the expression of PEDF protein in different ultrasound signs and different blood flow classification patients. Results The positive expression rate of PEDF protein in breast cancer tissues was 28.4% in study group, which was significantly lower than 65.5% in control group (P <0.05). The positive rates of PEDF protein in grade 0, grade 1, grade 2 and grade 3 were 50.5%, 38.8%, 29.6% and 10.9% respectively in color Doppler ultrasound blood flow. With the increase of blood flow grade, the positive rate of PEDF protein The lower the positive rate, the difference was statistically significant (P <0.05). The positive rate of PEDF protein in patients with tumor diameter ≤2.0 cm was 48.2%, which was significantly higher than 10.4% of tumor diameter> 2.0 cm, the difference was statistically significant (P <0.05). There was no significant difference in the positive expression rate of PEDF protein in patients with different morphology, borderline, posterior echo, aspect ratio, internal calcification, and BI-RADS classification (all P> 0.05). Conclusion Ultrasound signs of breast cancer can reflect the expression level of PEDF protein in tumor tissue to a certain extent, which has an important guiding significance for evaluating the prognosis of breast cancer patients.
