目的研究P21 ras、CD44v6、骨桥蛋白(OPN)在肺鳞癌和腺癌中的表达及其与肺鳞癌和腺癌预后的相互关系。方法应用免疫组化方法对96例非小细胞肺癌患者分别检测P21ras蛋白、CD44v6、OPN的表达,分析P21ras蛋白、CD44v6、OPN与肺癌组织学类型、分化程度、TNM分期等的关系,并通过34例随访病例了解以上指标与肺癌预后的关系。结果⑴中OPN及CD44v6在鳞癌中表达率高于腺癌,而P21ras蛋白则在鳞癌及腺癌间表达差异无统计学意义,但P21ras蛋白、CD44v6、OPN三者共同表达在鳞癌则高于腺癌;(2)晚期肺癌P21ras蛋白、CD44v6、OPN的阳性表达率均显著高于早期患者,而CD44v6及OPN则在淋巴结已有转移者阳性表达率显著高于无淋巴结转移者;(3)P21ras蛋白、CD44v6、OPN阳性组累积生存率低于阴性组。结论 P21ras蛋白、CD44v6、OPN在鳞癌三者共同高表达,三者共同作用可能与肺鳞癌的发生、发展密切相关,P21ras蛋白、CD44v6、OPN的阳性表达预示着非小细胞肺癌不良预后。
Objective To investigate the expression of P21 ras, CD44v6, osteopontin (OPN) in lung squamous cell carcinoma and adenocarcinoma and its relationship with the prognosis of lung squamous cell carcinoma and adenocarcinoma. Methods The expression of P21ras protein, CD44v6 and OPN were detected in 96 NSCLC patients by immunohistochemistry. The relationship between P21ras protein, CD44v6, OPN and the histological type, differentiation, and TNM staging of lung cancer was analyzed. Cases of follow-up patients understand the relationship between the above indicators and the prognosis of lung cancer. Results (1) The expression rates of OPN and CD44v6 in squamous cell carcinoma were higher than those in adenocarcinoma, but there was no significant difference in the expression of P21ras protein between squamous cell carcinoma and adenocarcinoma. However, the expression of P21ras protein, CD44v6 and OPN were jointly expressed in squamous cell carcinoma. The positive rate of P21ras protein, CD44v6, and OPN in advanced lung cancer were significantly higher than those in early stage, while the positive rate of CD44v6 and OPN in lymph node metastasis was significantly higher than that in no lymph node metastasis. 3) The cumulative survival rate of P21ras protein, CD44v6, OPN positive group was lower than that of negative group. Conclusion The expression of P21ras protein, CD44v6 and OPN are highly expressed in squamous cell carcinoma. The three effects may be closely related to the occurrence and development of lung squamous cell carcinoma. The positive expression of P21ras protein, CD44v6 and OPN predicts the poor prognosis of NSCLC.
