目的:探讨医院社区一体化管理模式对原发性高血压患者的防治效果。方法:选取2012年7月~2014年7月本院收治的500例高血压患者,随机将所有患者分为干预组和对照组,每组各250例。干预组患者采用医院社区一体化管理模式进行管理,对照组在医院进行常规治疗,不进行特殊干预。记录两组患者干预前后高血压的控制情况及高血压相关生活习惯控制情况。结果:干预后,干预组患者血压控制情况优于对照组,且高血压相关生活习惯控制情况也优于对照组,差异均有统计学意义(P<0.05)。结论:医院社区一体化管理模式对原发性高血压患者的防止效果显著,值得在基层医院和社区推广。
Objective: To explore the prevention and treatment effect of hospital community integrated management on patients with essential hypertension. Methods: From July 2012 to July 2014, 500 hypertensive patients admitted to our hospital were randomly divided into intervention group (n = 250) and control group (n = 250). Patients in the intervention group were managed by the hospital community integrated management mode, and the control group was routinely treated in the hospital without any special intervention. Record two groups of patients before and after the control of hypertension and hypertension-related lifestyle control situation. Results: After the intervention, the control of blood pressure in the intervention group was better than that in the control group, and the control of hypertension-related habits was also better than that of the control group (P <0.05). Conclusion: The hospital community integrated management mode has a significant preventive effect on patients with essential hypertension and is worth popularizing in primary hospitals and communities.
