目的 提高胃癌术前诊断的准确性 ,评价血管造影 (DSA)在胃癌浸润深度诊断中的作用 ;观察胃窦癌淋巴结转移规律与血供的关系。 方法 根据胃周的主要血管在进入胃壁各层的同时逐级分支的原理 ,通过识别走行于不同层次的血管所发生的变化判断胃癌的浸润深度。观察 10 1例胃癌患者血管造影结果 ,与手术及病理结果进行对比 ,得出准确率并进行评价 ;观察 35例经肝总动脉及腹腔动脉血管造影的胃窦癌的主要供血血管。 结果 10 1例患者血管造影结果对胃癌浸润深度诊断的准确率为 79 2 % ;胃窦癌的主要供血血管是胃网膜右动脉。 结论 血管造影对胃癌浸润深度诊断的准确性较高 ,为胃癌浸润深度的术前诊断提供了新的途径 ;胃窦癌的主要供血血管与淋巴结转移的主要方向存在着一致性 ,这可能是胃窦癌易出现幽门下淋巴结转移的原因。
Objective To improve the accuracy of preoperative diagnosis of gastric cancer, evaluate the role of angiography (DSA) in the diagnosis of gastric cancer invasion, and observe the relationship between lymph node metastasis and blood supply in gastric antrum cancer. Methods According to the principle that the main blood vessels of the stomach circumference branch into the layers of the stomach at the same time, the depth of invasion of gastric cancer can be judged by recognizing the changes that occur in blood vessels that travel at different levels. The angiographic results of 101 patients with gastric cancer were observed and compared with the results of surgery and pathology. The accuracy was evaluated and evaluated. The main blood supply vessels of 35 patients with gastric antral cancer who underwent angiography of common hepatic artery and celiac artery were observed. Results The diagnostic accuracy of angiographic results in 101 patients with gastric cancer was 79 2%. The main blood supply vessel of antral cancer was the right gastroepiploic artery. Conclusion The accuracy of angiography in diagnosing the depth of invasion of gastric cancer is relatively high, which provides a new approach for the preoperative diagnosis of invasive depth of gastric cancer. The main direction of the blood supply of gastric antrum cancer is consistent with the main direction of lymph node metastasis, which may be the stomach. Sinus cancer is prone to pyloric lymph node metastasis.
