目的:探讨重组人生长激素(rh GH)治疗糖尿病对血浆胰岛素样生长因子-1(IGF-1)及胰岛素样生长因子结合蛋白-3(IGFBP-3)水平的影响。方法:选取我院收治的80例糖尿病足患者,使用随机数字表法分为两组各40例,对照组实施常规治疗,观察组在常规治疗基础上加用rh GH,比较两组患者治疗前后空腹血糖(FPG)、血浆IGF-1、IGFBP-3水平变化以及临床疗效、溃疡愈合时间。结果:观察组治愈率为80.0%,显著高于对照组的37.5%(P<0.05);观察组溃疡平均愈合时间为(33.5±8.2)d,明显短于对照组的(44.5±6.7)d(P<0.05)。治疗后观察组的IGF-1、IGFBP-3、血清总蛋白、转铁蛋白水平分别为(216.33±21.47)ng·ml-1、(45.96±5.27)ng·ml-1、(72.85±3.52)g·L-1、(2.94±0.21)g·L-1,均显著高于对照组的(183.74±13.29)ng·ml-1、(38.62±5.87)ng·ml-1、(59.54±4.59)g·L-1、(2.11±0.27)g·L-1(P<0.05)。两组治疗后的FPG水平均较治疗前显著降低,但组间比较差异无统计学意义(P>0.05)。结论:rh GH治疗糖尿病足能够有效促进血浆IGF-1及IGFBP-3水平上升,加快创面愈合,提高创面愈合率,缩短创面愈合时间。
Objective: To investigate the effect of recombinant human growth hormone (rh GH) on plasma insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) Methods: 80 cases of diabetic foot admitted to our hospital were randomly divided into two groups of 40 cases using random number table. The control group was given routine treatment. The observation group was given rh GH on the basis of routine treatment. Fasting blood glucose (FPG), plasma IGF-1, IGFBP-3 levels and clinical efficacy, ulcer healing time. Results: The cure rate of the observation group was 80.0%, which was significantly higher than that of the control group (37.5%, P <0.05). The average healing time of the ulcer in the observation group was (33.5 ± 8.2) d, which was significantly shorter than that of the control group (44.5 ± 6.7) d (P <0.05). After treatment, the levels of IGF-1, IGFBP-3, serum total protein and transferrin in the observation group were (216.33 ± 21.47) ng · ml-1, (45.96 ± 5.27) ng · ml- g · L-1 and (2.94 ± 0.21) g · L-1 in the control group were significantly higher than those in the control group (183.74 ± 13.29 ng · ml-1, 38.62 ± 5.87 ng · ml-1, 59.54 ± 4.59 ) g · L-1, (2.11 ± 0.27) g · L-1 (P <0.05). FPG levels after treatment in both groups were significantly lower than before treatment, but there was no significant difference between the two groups (P> 0.05). Conclusion: rhGH treatment of diabetic foot can effectively promote the increase of plasma IGF-1 and IGFBP-3 levels, accelerate wound healing, improve wound healing rate and shorten wound healing time.
