目的应用心脏磁共振成像检测青壮年肥胖者心脏结构及功能的改变。方法对青壮年肥胖者15例和16例健康志愿者(对照组),应用1.5T超导型磁共振扫描仪(Sonata,Siemens),体部相控阵及表面线圈,胸前导联回顾性心电门控,采集左心室长轴两腔位、四腔位、三腔位及短轴位动态图像,采用TurboFlash2D电影序列行左室短轴电影成像,用argus软件mass analysis分别描记左室心内、外膜界面,自动画出左室容积-时间变化曲线,测得左心室舒张末期容积参数和功能参数,并对正常志愿者组与肥胖者组临床资料及左心室功能数据结果进行比较。结果肥胖组同健康志愿者组比较,左室舒张末期容积(EDV)、收缩末期容积(ESV)、每搏量(SV)、心输出量(CO)、心肌质量(MM),以及左室高峰射血率(PER)、高峰射血时间(TPER)、高峰充盈率(PFR)、高峰充盈时间(TPFR)、室壁增厚率(TN),差异均有统计学意义(P<0.05),射血分数(EF)及第一层增厚率(P>0.05),无统计学差异。结论肥胖可引起青壮年人群心脏结构改变,使青壮年人群心脏舒张功能下降;CMR还可以较早检测肥胖者心脏结构和功能的异常。
Objective To evaluate the cardiac structure and function of obese young adults with cardiac magnetic resonance imaging. Methods Fifteen cases of obesity in young adults and 16 healthy volunteers (control group) were retrospectively analyzed with 1.5T superconducting magnetic resonance scanner (Sonata, Siemens), body phased array and surface coil. ECG gating, the acquisition of left ventricular long axis two-chamber, four-chamber, three-chamber and short axis dynamic images, the use of TurboFlash2D movie sequence of left ventricular short axis film imaging, argus software mass analysis were recorded left ventricular The left ventricular volume-time curves were automatically plotted on the inner and outer membrane interfaces. The left ventricular end-diastolic volumetric parameters and functional parameters were measured. The clinical data and left ventricular function data were compared between normal volunteers and obese subjects. Results Compared with healthy volunteers, the left ventricular end diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), cardiac output (CO), myocardial mass (MM) There were significant differences in PER, TPER, PFR, TPFR, TN between the two groups (P <0.05) Ejection fraction (EF) and the first layer of thickening rate (P> 0.05), no statistical difference. Conclusion Obese can cause changes of cardiac structure in young and middle-aged people, and decrease cardiac diastolic function in young and middle-aged people. CMR can also detect heart structure and function abnormalities in obese people earlier.
