目的:应用无赤光眼底照相及图像处理技术观察视网膜神经纤维层(RNFL)形态,并与视野检查结果进行对比分析,探讨RNFL检查在青光眼早期诊断中的临床意义。方法:选30例(55眼)门诊可疑的青光眼患者,进行无赤光眼底照相及视野检查,经Photoshop图像处理软件对眼底图像进行处理后观察,并与视野检查结果进行统计学检验及对比分析。结果:以无赤光眼底照相及图像处理技术观察RNFL形态方法的敏感性为94%,阳性率为78%,视野检查的阳性率为67%,经卡方检验,差异有统计学意义(χ2=15.0349,P=0.0001),提示RNFL观察的阳性率高于视野检查,结合临床,认为RN-FL观察法实际上是较视野检查更早的发现了神经纤维的损害。结论:应用无赤光眼底照相及图像处理技术观察RNFL形态,灵敏度较高,能够较视野检查更早的发现神经纤维的损害,为青光眼早期诊断、随访监测及治疗方案提供参考,且方法简单易行,适于普及,值得在青光眼早期诊断中推广应用。
OBJECTIVE: To observe the morphology of retinal nerve fiber layer (RNFL) using naked retinal photocoagulation and image processing techniques and compare with the visual field test results to explore the clinical significance of RNFL examination in the early diagnosis of glaucoma. Methods: Thirty patients (55 eyes) with suspected glaucoma were selected for retina without fundus photography and visual field examination. After fundus images were processed by Photoshop image processing software, the results were compared with the visual field test results for statistical analysis and comparative analysis . Results: The sensitivity of RNFL morphology was 94%, the positive rate was 78% and the positive rate of visual field examination was 67%. The chi square test showed that the difference was statistically significant (χ2 = 15.0349, P = 0.0001), suggesting that the positive rate of RNFL observation was higher than that of visual field examination. Combined with clinical observation, it is considered that RN-FL observation method actually found nerve fiber damage earlier than visual field examination. CONCLUSION: The naked eye photocoagulation and image processing techniques are used to observe the morphology of RNFL. It has high sensitivity and can detect the damage of nerve fibers earlier than the visual field examination. It provides a reference for the early diagnosis, follow-up monitoring and treatment of glaucoma. The method is simple and easy Line, suitable for universal, it is worth to promote the early diagnosis of glaucoma.
