目的探讨绝经后女性唾液皮质醇水平与代谢综合征(metabolicsyndrome,MS)的关系。方法 2014年8月—2016年5月,选取120例绝经后女性MS患者作为MS组,另选110例绝经后健康女性作为对照组,进行血压、身高、体质量测量,计算体重指数(body mass index,BMI)。检测两组唾液皮质醇以及血清三酰甘油(triglyceride,TG)、胆固醇(cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDLC)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDLC)、空腹血糖(fasting blood glucose,FBG)及空腹胰岛素(fasting insulin,FINS)水平,用稳态模型评估法(homeostatic model assessment,HOMA)来评价胰岛素抵抗指数(HOMA-IR)。计量资料采用t检验,相关分析采用Pearson相关分析,P<0.05为差异有统计学意义。结果 MS组唾液皮质醇水平、BMI、收缩压、FBG、TG、FINS、HOMA-IR水平分别为(12.21±5.72)mmol/L、(26.6±2.2)kg/m2、(129±10)mm Hg(1 mm Hg=0.133k Pa)、(6.31±1.32)、(2.12±1.20)mmol/L、(57.2±26.7)pmol/L、(5.36±1.41),均显著高于对照组的(10.33±3.54)mmol/L、(22.6±1.6)kg/m2、(114±7)mm Hg、(5.1±0.4)、(1.57±0.83)mmol/L、(43.9±22.1)pmol/L、(2.59±1.08),差异均有统计学意义(均P<0.05)。MS组HDL-C水平为(1.35±0.34)mmol/L,明显低于对照组的(1.64±0.25)mmol/L,差异有统计学意义(P<0.05)。MS组唾液皮质醇与BMI、舒张压、TG、FINS及HOMA-IR呈正显著相关(r=0.695、0.412、0.649、0.706、0.587、0.352),与HDL-C呈显著负相关(r=-0.614)。结论绝经后女性唾液皮质醇水平与MS呈正相关。
Objective To investigate the relationship between salivary cortisol and metabolic syndrome (MS) in postmenopausal women. Methods From August 2014 to May 2016, 120 MS patients with postmenopausal MS were selected as the MS group, and 110 healthy postmenopausal women were selected as the control group. Blood pressure, height and weight were measured and body mass index index, BMI). The levels of salivary cortisol and triglyceride (TG), cholesterol (TC), low density lipoprotein cholesterol (LDLC), high density lipoprotein cholesterol (HDLC) ), Fasting blood glucose (FBG) and fasting insulin (FINS) were measured. The HOMA-IR was evaluated by homeostatic model assessment (HOMA). Measurement data using t test, correlation analysis using Pearson correlation analysis, P <0.05 for the difference was statistically significant. Results The levels of salivary cortisol, BMI, systolic blood pressure, FBG, TG, FINS and HOMA-IR in saliva were (12.21 ± 5.72) mmol / L and (26.6 ± 2.2) kg / (6.31 ± 1.32), (2.12 ± 1.20) mmol / L, (57.2 ± 26.7) pmol / L and (5.36 ± 1.41) in the control group were significantly higher than those in the control group (10.33 ± 3.54 mmol / L, 22.6 ± 1.6 kg / m2, 114 ± 7 mm Hg, 5.1 ± 0.4, 1.57 ± 0.83 mmol / L and 43.9 ± 22.1 pmol / 1.08), the differences were statistically significant (all P <0.05). The level of HDL-C in MS group was (1.35 ± 0.34) mmol / L, which was significantly lower than that in control group (1.64 ± 0.25) mmol / L, the difference was statistically significant (P <0.05). There was a significant positive correlation between salivary cortisol and BMI, diastolic blood pressure, TG, FINS and HOMA-IR (r = 0.695,0.412,0.649,0.706,0.587,0.352) in MS group, but negatively correlated with HDL-C ). Conclusions Salivary cortisol levels in postmenopausal women are positively correlated with MS.
