目的探讨软骨肉瘤组织中Fascin蛋白的水平变化及临床意义。方法选取2009年1月至2016年1月间南京医科大学第一附属医院收治的92例软骨肉瘤患者为观察组,选取同期67例软骨瘤患者为对照组。采用免疫组化EnlivionTM法分别检测两组患者组织中Fascin蛋白表达,并分析Fascin蛋白水平变化与软骨肉瘤临床病理特征之间的关系。结果软骨肉瘤组织中的Fascin蛋白阳性率为70.7%,软骨瘤组织中为4.5%,两组比较,差异有统计学意义(P<0.05)。软骨肉瘤组织中,Fascin蛋白阳性率与患者年龄、性别无显著相关性,差异无统计学意义(P>0.05);在不同病理类型中,间叶型、透明细胞型与普通型组织的Fascin蛋白阳性率表达无明显差异,但去分化型软骨肉瘤Fascin蛋白阳性率91.7%明显高于普通型的78.9%,组间差异有统计学意义(P<0.05);在普通型软骨肉瘤组织学分级中,Ⅰ级阳性表达率为47.6%,Ⅱ级为63.2%,Ⅲ级为88.2%,组间差异有统计学意义(P<0.05);Fascin蛋白在肺转移组中的表达为87.2%,明显高于无肺转移组的48.9%,组间差异有统计学意义(P<0.05)。结论 Fascin蛋白参与了软骨肉瘤恶变的发生、发展过程,其高表达水平与软骨肉瘤的分化、转移密切有关,可作为临床治疗软骨肉瘤的新方向。
Objective To investigate the changes of Fascin protein in chondrosarcoma and its clinical significance. Methods A total of 92 patients with chondrosarcoma admitted to the First Affiliated Hospital of Nanjing Medical University from January 2009 to January 2016 were selected as the observation group and 67 patients with chondroma were enrolled as the control group. The expression of Fascin protein was detected by immunohistochemical EnlivionTM method in two groups of patients, and the relationship between Fascin protein level and clinicopathological features of chondrosarcoma was analyzed. Results The positive rate of Fascin in chondrosarcoma was 70.7% and in chondroma was 4.5%. There was significant difference between the two groups (P <0.05). The positive rate of Fascin protein in chondrosarcoma tissue was not significantly correlated with age and sex of the patients (P> 0.05). In different pathological types, Fascin protein of mesenchyme, clear cell type and common type The positive rate of Fascin protein in dedifferentiated chondrosarcoma was 91.7%, which was significantly higher than 78.9% in common type (P <0.05). In the histological grade of common chondrosarcoma , The positive expression rate of grade Ⅰ was 47.6%, grade Ⅱ was 63.2%, grade Ⅲ was 88.2%, the difference was statistically significant (P <0.05); the expression of Fascin protein in lung metastasis was 87.2% There was a significant difference between the two groups (P <0.05) in 48.9% of patients without lung metastasis. Conclusions Fascin protein is involved in the occurrence and development of malignant chondrosarcoma. The high level of expression of Fascin is associated with the differentiation and metastasis of chondrosarcoma, which may serve as a new direction for clinical treatment of chondrosarcoma.
