目的探讨早产儿肺功能与神经行为发育之间的关系。方法选取新生儿科无明显脑损伤的55例早产儿,排除中枢神经系统感染、窒息、先天畸形等可能导致神经学发育异常的疾病。矫正胎龄40周时行52项神经运行检查,根据结果分为正常组及异常组,并行潮气呼吸肺功能检查,比较两组早产儿肺功能的特点及差异。结果异常组早产儿呼吸频率(RR)、每分钟通气量明显高于正常组,达峰时间比(TPIEF/TE)、达峰容积比(VPEF/VE)、呼气峰流速(TPEF)及75%、50%、25%潮气量时呼气流速(TEF75、TEF50、TEF25)均明显低于正常组(P<0.01)。结论无明显脑损伤的早产儿也可能存在神经行为发育异常,且可能与发育不成熟的肺功能相关。
Objective To investigate the relationship between pulmonary function and neurobehavioral development in preterm infants. Methods 55 cases of premature infants with no obvious brain injury in neonatology were selected to exclude the diseases that may lead to neurological dysplasia such as central nervous system infection, asphyxia and congenital malformations. Fifty-two neurological examinations were performed at 40 weeks after correction of gestational age. According to the results, the patients were divided into normal group and abnormal group. The pulmonary function tests were carried out in order to compare the characteristics and differences of lung function in preterm infants. Results The respiratory rate (RR) and the minute ventilation in the abnormal group were significantly higher than those in the normal group (P <0.05), and the ratio of peak time to volume (VPEF / VE), peak expiratory flow rate (TPEF) The expiratory flow rate (TEF75, TEF50, TEF25) in%, 50%, 25% tidal volume were significantly lower than those in normal group (P <0.01). Conclusions There may be abnormal neurobehavioral abnormalities in premature infants without obvious brain injury and may be related to immature pulmonary function.
