目的:探查疮灵液对慢性创面肉芽金属蛋白酶表达的影响及促进创面愈合的部分作用机制。方法:采用大鼠背部创面金黄色葡萄球菌液注射模型,实验分为疮灵液组与对照组,疮灵液组以疮灵液覆盖创面,对照组以生理盐水纱布覆盖创面,隔日换药,术时、术后3 d、术后7 d、术后14 d创面照相测面积,取分泌物测TNF-α、IL-6含量及粒细胞总数,取肉芽测金属蛋白酶(MMP1、MMP2、MMP9)及其抑制剂(TIMP1)与羟脯氨酸(Hydroxyproline,Hyp)含量。结果:术后3~14 d,疮灵液组创面面积显著小于对照组,具有极显著统计学差异(P<0.01);疮灵液组创面分泌物中白细胞总数均显著少于对照组,14 d时,疮灵液组白细胞总数接近正常;疮灵液组创面分泌物中TNF-α、IL-6含量于术后7 d恢复正常,均显著低于对照组;术后3 d起,创面肉芽内MMP1、MMP2及MMP9均显著低于对照组(P<0.01),其中以MMP9表达影响疗效最显著,疮灵液对TIMP1未见明显影响;创灵液组在术后7、14 d肉芽中羟脯氨酸含量升高较明显,与对照组相比,有显著性差异。结论:疮灵液能够下调肉芽中金属蛋白酶含量,抑制慢性创面分泌物炎症因子分泌,并能够调节创面羟脯氨酸含量,从而促进慢性创面愈合。
Objective: To investigate the effect of soothing spleen fluid on the expression of granulation metalloproteinases in chronic wounds and the mechanism of partial wound healing. Methods: Staphylococcus aureus fluid injection model was established on the back of rats. The experiment was divided into sling group and control group. The sore liquid group covered the wound with sore liquid and the control group covered the wound with saline gauze. At 3 days after operation, 7 days postoperatively and 14 days postoperatively, the area of wounds was measured and the contents of TNF-α, IL-6 and the total number of granulocytes were measured. The expressions of MMP1, MMP2, MMP9 ) And its inhibitors (TIMP1) and hydroxyproline (Hydroxyproline, Hyp) content. Results: The area of wounds in the sling solution group was significantly smaller than that in the control group from 3 to 14 days after operation (P <0.01). The total number of leukocytes in the wound secretions of sling solution group was significantly less than that of the control group d, the total number of leukocytes in the soothing fluid group was close to normal. The contents of TNF-α and IL-6 in the wound secretions of soothing liquid group returned to normal at 7 days after operation, both of which were significantly lower than those in the control group. Granulation in the MMP1, MMP2 and MMP9 were significantly lower than the control group (P <0.01), of which the effect of MMP9 expression the most significant effect, soothing fluid on TIMP1 no significant effect; Chuangling solution group at 7,14 d postoperative granulation Hydroxyproline content increased significantly, compared with the control group, there was a significant difference. Conclusion: SAG can down-regulate the content of metalloproteinases in granulation, inhibit the secretion of inflammatory factors in chronic wound secretions, and can regulate the content of hydroxyproline in the wound surface, thus promoting the healing of chronic wounds.
