目的探讨二维超声对妊娠期糖尿病胎儿心脏发育的测量价值。方法选取血糖控制良好的妊娠期糖尿病孕妇胎儿39例,正常妊娠胎儿106例作为对照组。运用二维超声测量胎儿的心脏横径、胸廓横径、左右心房及左右心室横径、主动脉内径、肺动脉内径、主动脉峡部、降主动脉及动脉导管内径。结果血糖控制良好的妊娠期糖尿病孕妇胎儿和正常妊娠胎儿心脏横径、胸廓横径、左右心房及左右心室横径、主动脉内径、肺动脉内径、主动脉峡部、降主动脉及动脉导管内径均随孕周的增加而增长。两组各测量指标相比,差异均无显著性,P>0.05。血糖控制良好的妊娠期糖尿病孕妇胎儿和正常妊娠胎儿肺动脉内径大于主动脉内径,右心房横径大于左心房横径,P<0.05;右心室横径与左心室横径相比差异无显著性,P>0.05。结论妊娠期糖尿病孕妇若血糖控制良好,则胎儿心脏结构无明显变化,胎儿心脏生长发育规律与正常妊娠胎儿一样,随孕周增长逐渐成熟。
Objective To investigate the value of two-dimensional ultrasound in fetal cardiac development of gestational diabetes mellitus. Methods Totally 39 fetuses with gestational diabetes mellitus with well-controlled blood glucose control and 106 fetuses with normal pregnancy were selected as the control group. Using two-dimensional ultrasound to measure fetal heart diameter, thoracic transverse diameter, left and right atrium and left ventricular diameter, aortic diameter, pulmonary artery diameter, aortic isthmus, descending aorta and arterial catheter diameter. Results Fetal gestational diabetes mellitus with gestational diabetes mellitus and normal fetuses had a significant positive correlation with fetal cardiac diameter, thoracic transverse diameter, left and right atrium and left ventricular diameter, aortic diameter, pulmonary artery diameter, aortic isthmus, descending aorta and arterial catheter diameter Increase in gestational age and growth. There was no significant difference between the two groups of measurement indexes, P> 0.05. Blood glucose control of pregnant women with gestational diabetes mellitus and normal fetus fetal pulmonary artery diameter larger than the aortic diameter, right atrial diameter greater than the diameter of the left atrium, P <0.05; right ventricular diameter and left ventricular diameter compared to no significant difference, P> 0.05. Conclusion Pregnancy in gestational diabetes, if blood glucose control is good, the fetal heart structure no significant changes in fetal heart growth and development of the fetus and normal pregnancy, with the growth of gestational age gradually matured.
