目的探究在孕11~13~(+6)周系统超声检查对于早期胎儿畸形的诊断效果及意义。方法行11~13~(+6)周早期系统超声检查孕妇4575例,对其实施孕11~13~(+6)周胎儿系统超声检查,测量并记录以超声指标,胎儿顶臀径、双顶径、股骨及肱骨长度、颈部透明带厚度(NT)、鼻骨长度、静脉导管血流频谱、三尖瓣血流频谱,观察胎儿结构畸形状况,超声提示结构畸形的胎儿告知胎儿父母预后情况,让他们决定是否终止妊娠,余病例继续妊娠,行中孕期系统超声筛查并追踪其妊娠结局。结果在4575例孕11~13~(+6)周孕妇中,胎儿畸形79例,胎儿畸形发生率为1.73%,早孕期超声检查胎儿异常46例,检出率为58.23%。其中前腹壁畸形5例,骨骼、四肢异常1例,神经系统异常1例,颈部NT增厚及颈部水囊瘤26例,全前脑2例,心血管异常2例,颜面部畸形6例,其他异常1例。结论孕11~13~(+6)周实施系统超声检查对胎儿畸形的筛查具有较高的临床价值,可帮助早期及时发现胎儿结构畸形状况,减少引产对孕妇超成的伤害,为产前咨询及临床治疗提供依据,值得临床推广。
Objective To investigate the diagnostic value and significance of systematic ultrasonography for early fetal malformation at 11 ~ 13 ~ (+6) weeks of gestation. Methods 4575 pregnant women at 11 ~ 13 ~ (+6) weeks’ early stage were enrolled in this study. The fetuses of 11 ~ 13 ~ (+6) weeks pregnant were enrolled in this study. Results Among 4575 pregnant women with 11 ~ 13 ~ (+6) weeks of pregnancy, 79 cases of fetal malformations and fetal malformations accounted for 1.73%. 46 cases of fetal abnormalities in early pregnancy were detected by ultrasound and the detection rate was 58.23%. Among them, there were 5 cases of anterior abdominal wall deformity, 1 case of skeletal and extremity anomaly, 1 case of nervous system abnormality, 26 cases of thickened neck and 26 cases of neck cystadenocarcinoma, 2 cases of anterior forebrain, 2 cases of cardiovascular abnormalities, 6 cases of facial deformities Cases, other abnormalities in 1 case. Conclusion The screening of fetus malformations during 11 ~ 13 ~ (+6) weeks pregnant with ultrasound is of high clinical value, which can help early detection of fetal structural malformations in an early stage and reduce the damage caused by induced abortion. Counseling and clinical treatment to provide the basis, it is worth clinical promotion.
