目的研究2型糖尿病患者血清抵抗素、瘦素、脂联素水平与颈动脉内中膜厚度及斑块稳定性的关系。方法纳入2型糖尿病患者60例、糖耐量异常患者50例、健康体检人群50例,用酶联免疫吸附试验(ELISA)法检测血清抵抗素、瘦素、脂联素水平,并检测颈动脉内中膜厚度(IMT)及斑块形成率。结果耐糖量异常、2型糖尿病患者血清抵抗素水平分别为(3.26±1.85),(4.61±2.08)ng·mL~(-1),瘦素水平分别为(1.83±0.09),(3.10±0.15)ng·mL~(-1)、脂联素水平分别为(3.57±0.11),(1.61±0.12)μg·mL~(-1)、IMT分别为(1.15±0.04),(1.59±0.12)mm,颈动脉斑块检出率分别为10.00%(5/50例)和51.67%(31/60例),差异均有统计学意义(P<0.05);2型糖尿病患者中无斑块组、稳定性斑块组及不稳定斑块组血清抵抗素水平分别为(3.01±1.25),(4.17±1.08),(5.35±1.16)ng·mL~(-1)、瘦素水平分别为(1.33±0.15),(2.21±0.12),(3.15±0.17)ng·mL~(-1),而脂联素水平分别为(2.81±0.10),(1.95±0.09),(1.14±0.12)μg·mL~(-1),差异均有统计学意义(P<0.05);血清抵抗素与瘦素呈正相关(P<0.01),二者与APN水平均呈负相关(P<0.01)。结论 2型糖尿病患者血清抵抗素、瘦素水平升高、脂联素水平降低与颈动脉内中膜厚度增厚、斑块形成及稳定性密切相关。
Objective To study the relationship between serum resistin, leptin and adiponectin levels and carotid intima-media thickness and plaque stability in type 2 diabetic patients. Methods Sixty patients with type 2 diabetes mellitus, 50 patients with impaired glucose tolerance and 50 healthy people were enrolled in this study. Serum resistin, leptin and adiponectin levels were measured by enzyme-linked immunosorbent assay (ELISA) Media thickness (IMT) and plaque formation rate. Results The level of serum resistin was (3.26 ± 1.85), (4.61 ± 2.08) ng · mL -1 in patients with type 2 diabetes mellitus, the levels of leptin were (1.83 ± 0.09) and (3.10 ± 0.15 (3.57 ± 0.11) and (1.61 ± 0.12) μg · mL ~ (-1), IMT were (1.15 ± 0.04) and (1.59 ± 0.12), respectively mm, carotid plaque detection rate was 10.00% (5/50 cases) and 51.67% (31/60 cases), the differences were statistically significant (P <0.05); type 2 diabetes patients without plaque group (3.01 ± 1.25), (4.17 ± 1.08) and (5.35 ± 1.16) ng · mL ~ (-1) in stable plaque group and unstable plaque group respectively. The levels of leptin were ( 1.33 ± 0.15, 2.21 ± 0.12 and 3.15 ± 0.17 ng · mL -1, while the adiponectin levels were (2.81 ± 0.10), (1.95 ± 0.09) and (1.14 ± 0.12) μg · ML -1, the difference was statistically significant (P <0.05). Serum resistin was positively correlated with leptin (P <0.01), both of them were negatively correlated with APN (P <0.01). Conclusions Serum resistin and leptin levels are elevated in patients with type 2 diabetes. The decrease of adiponectin level is closely related to the thickening of carotid intima - media thickness, plaque formation and stability.
