目的:探讨18F-FDGPET/CT在诊断肺部占位性病变中的价值。方法:对100例肺部占位性病变进行回顾性分析,比较PET/CT与PET及CT的诊断效度、不同大小肿瘤的SUV、不同病理类型肺癌的SUV。结果:①PET/CT的灵敏度、阴性预测值、准确率优于CT(P<0.01),PET/CT的特异度优于PET(P<0.05)。②依据肿瘤大小分为<3cm、3~5cm、>5cm组,应用单向方差分析对比不同组间SUV,3组间SUV均有差异,P<0.01。各组间SUV两两比较,发现小于3cm组SUV<3~5cm组SUV<5cm以上组SUV。③鳞癌组与腺癌组SUV无显著差异(P=0.821,P>0.05)。结论:18F-FDGPET/CT在诊断肺部占位性病变中将PET代谢学特征密切结合CT的形态学表现,可大大提高各项诊断效率,达到优势互补的目的。
Objective: To investigate the value of 18F-FDG PET / CT in the diagnosis of lung space-occupying lesions. Methods: 100 cases of lung space occupying lesions were retrospectively analyzed. The diagnostic validity of PET / CT, PET and CT, SUV of different sizes and SUV of different pathological types of lung cancer were compared. Results: ①The sensitivity and negative predictive value of PET / CT were better than those of CT (P <0.01). The specificity of PET / CT was better than PET (P <0.05). According to the tumor size, the SUVs were divided into 3cm, 3 ~ 5cm and 5cm groups. SUVs among different groups were compared by one-way analysis of variance (ANOVA). SUV between the two groups compared, found less than 3cm group SUV <3 ~ 5cm group SUV <5cm above group SUV. ③ There was no significant difference in SUV between squamous cell carcinoma and adenocarcinoma (P = 0.821, P> 0.05). Conclusion: 18F-FDG PET / CT in the diagnosis of lung space-occupying lesions will PET metabolic characteristics closely combined with the morphological features of CT, which can greatly improve the diagnostic efficiency and achieve the purpose of complementarity.
