目的 调查不同出生体重儿成年后代谢异常的现患率,探索两者的关系。方法 对上海市平凉社区糖尿病流行病学调查中15 ~74岁有确切出生体重记录的1 010名中国人(男性390例,女性620例)检测体重指数、腰围、臀围、腰臀比、血压、空腹血糖、胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇。以单变量及多变量分析方法观察不同出生体重儿成年后代谢异常的差别及两者的关系。结果 低出生体重者( <2 5kg) 56人,高出生体重者(≥4kg) 53人、正常出生体重者901人。低出生体重组成年后BMI均高于正常出生体重组,其中女性体质指数和腰围、腰臀比两组差异有统计学意义(P<0. 01 )。男性高出生体重组成年后腰围和臀围均较正常出生体重组高,两组差异有统计学意义(P<0 .05);低出生体重组成年后肥胖、高胆固醇血症、糖尿病及糖尿病合并高血压的患病率均高于正常出生体重组(P<0 .05、P<0 .01、P<0 05、P<0. 01),高出生体重组成年后肥胖发生率和高血压患病率均高于正常出生体重组(P<0 .05);低出生体重与成年后超重肥胖、腹型肥胖、高胆固醇血症及糖尿病的发病呈显著性相关。结论 低出生体重与成年后肥胖、糖脂代谢异常相关,是成年后发生超重肥胖、高胆固醇血症和糖尿病的危险因素。
Objective To investigate the prevalence of metabolic abnormalities among different birth weight infants and explore their relationship. Methods A total of 1 010 Chinese (390 males and 620 females) with accurate birth weight records aged 15-74 years in Pingliang Community Diabetes Epidemiology Survey of Shanghai City were tested for body mass index, waist circumference, hip circumference, waist-hip ratio, blood pressure , Fasting blood glucose, cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol. Univariate and multivariate analysis were used to observe the differences in metabolic abnormalities among adults with different birth weights and their relationship. Results Low birth weight (<2 5kg) 56 people, high birth weight (≥ 4kg) 53 people, normal birth weight 901 people. BMI in low birth weight group was higher than that in normal birth weight group, and there was significant difference between female body mass index, waist circumference and waist-hip ratio in both groups (P <0.01). Compared with normal birth weight group, waist circumference and hip circumference in male with high birth weight were significantly higher than those in normal birth weight group (P <0.05); low birth weight group had obesity, hypercholesterolemia, diabetes and diabetes after adulthood The prevalence of hypertension was higher than that of normal birth weight group (P <0.05, P <0.01, P <0.05, P <0.01), and the incidence of obesity was higher in high birth weight adults The prevalence of blood pressure was higher than that of normal birth weight group (P <0.05). Low birth weight was significantly associated with the incidence of overweight and obesity, abdominal obesity, hypercholesterolemia and diabetes in adulthood. Conclusion Low birth weight is associated with obesity and abnormal glucose and lipid metabolism in adult. It is a risk factor for overweight and obesity, hypercholesterolemia and diabetes in adulthood.
