目的分析我院门诊药房内部调剂差错事件的原因,采取适当干预措施,以提高发药准确率,提升医疗服务质量。方法对我院门诊药房2010年10月至2011年3月的内部调剂差错进行汇总,分析其类型及原因并采取相应干预措施。采取干预措施后再与2011年10月至2012年3月的内部调剂差错对比。结果通过采取干预措施,内部调剂差错由230例降至194例,降低了15.65%,发生率干预前后分别为0.055 9%与0.037 4%,存在明显差异。结论适当的干预措施有利于减少内部调剂差错,提高医疗服务质量,保证患者用药安全。
Objective To analyze the causes of errors in the internal pharmacy of our hospital and take appropriate interventions to improve the accuracy of drug delivery and improve the quality of medical services. Methods To summarize the internal adjustment errors of outpatient pharmacy in our hospital from October 2010 to March 2011, analyze the types and causes and take corresponding intervention measures. Interventions were followed by errors compared to internal transfers from October 2011 to March 2012. Results Interventions were taken to reduce the internal adjustment error from 230 cases to 194 cases and a decrease of 15.65%. There were significant differences between the intervention rates of 0.055 9% and 0.037 4% before and after the intervention. Conclusion Appropriate interventions will help to reduce the internal transfer errors, improve the quality of medical services and ensure the safety of patients.
