目的探讨数字减影CT血管成像(DSCTA)在血管性病变诊断中的应用价值。方法收集在本院行DSCTA检查的血管性疾病50例,使用西门子公司Emotion 16层螺旋CT机,螺距0.9~1.5,层厚0.75~1.5 mm,注药流率2.5~4.0 ml/s,成人剂量90~100 ml,儿童按2 ml/kg计算,扫描延迟时间用智能跟踪法扫描。图像后处理使用西门子公司Emotion工作站进行血管三维重建。结果DSCTA三维成像可以立体、直观地显示血管的空间解剖细节,可充分观察到动脉瘤的准确起源、瘤体大小、颈部宽窄及走向等情况,本组病例中所显示的最小动脉瘤直径约3 mm;同时,DSCTA也可作为动脉瘤术后评价的一种无创性检查方法,观察术后银夹所在的位置和远端血管通畅情况等。三维图像对于动静脉畸形、血管狭窄及颅内静脉系统疾病也具有良好的诊断价值。在重建成像中,容积再现图像质量最佳。结论DSCTA是一种无创性成像方法,在血管性疾病的诊断和术前术后评价中发挥着重要作用。
Objective To investigate the value of digital subtraction CT angiography (DSCTA) in the diagnosis of vascular lesions. Methods Fifty cases of vascular diseases were collected by DSCTA in our hospital. The samples were collected on a Siemens Emotion 16-slice spiral CT machine with a pitch of 0.9-1.5 and a thickness of 0.75-1.5 mm. The injection rate was 2.5-4.0 ml / ~ 100 ml, children according to 2 ml / kg calculation, scan delay time with intelligent tracking method. Image postprocessing Three-dimensional reconstruction of blood vessels was performed using the Siemens Emotion workstation. Results Three-dimensional imaging by DSCTA could display the spatial anatomic details of blood vessels in a stereoscopic and visual manner. The exact origin of the aneurysms, the size of the tumor, the width and the neck of the neck, etc. were observed. The diameter of the smallest aneurysm shown in this group was about 3 mm. At the same time, DSCTA can also be used as a noninvasive method for the postoperative evaluation of aneurysms. The position of the silver clip and the patency of the distal blood vessel were observed. Three-dimensional images for arteriovenous malformations, vascular stenosis and intracranial venous system disease also has a good diagnostic value. Volumetric reconstructed images are best in reconstructed images. Conclusion DSCTA is a noninvasive imaging method that plays an important role in the diagnosis and preoperative and postoperative evaluation of vascular diseases.
