目的探讨脑钠肽(BNP)水平与原发性高血压严重程度的相关性。方法选择原发性高血压患者354例作为高血压组,同期来某院的健康体检者100例作为健康对照组,测定室间隔厚度(leftventricularseptumthickness,LVST)、左心室后壁厚度(leftventricularposteriorwallthickness,LVPWT)、左室舒张末内径(leftventricularenddiastolicdiame-ter,LVEDD)和血浆BNP浓度。结果高血压组血浆中BNP水平较健康对照组明显升高,差异有统计学意义(P﹤0.05),并随血压分级升高而升高;BNP水平与LVST、LVPWT、LVEDD呈显著正相关,差异有统计学意义(r=0.51,P﹤0.05;r=0.46,P﹤0.05;r=0.40,P﹤0.01);同级高血压组中伴左室肥厚组血浆BNP水平较无左室肥厚组显著升高(P﹤0.05);在同级别高血压组中按照危险分层进行亚组分析得出随危险分层升高,BNP水平亦升高,差异有统计学意义(P﹤0.05)。结论原发性高血压患者的血浆BNP水平较健康对照组显著升高,BNP水平与血压分级密切相关,与左室肥厚程度成正相关,并随危险分层的升高而升高。BNP有可能成为原发性高血压危险分层的指标。
Objective To investigate the correlation between the level of brain natriuretic peptide (BNP) and the severity of essential hypertension. Methods 354 patients with essential hypertension were selected as hypertensive group. One hundred healthy volunteers from a hospital during the same period were selected as healthy control group. The left ventricular tachycardia thickness (LVST), left ventricular posterior wall thickness (LVPWT) , Left ventricular end-diastolic diameter (LVEDD) and plasma BNP concentration. Results Plasma BNP level in hypertension group was significantly higher than that in healthy control group (P <0.05), and increased with the increase of blood pressure level. BNP level was positively correlated with LVST, LVPWT and LVEDD, The difference was statistically significant (r = 0.51, P <0.05; r = 0.46, P <0.05; r = 0.40, P <0.01); in patients with left ventricular hypertrophy (P <0.05). Subgroup analysis according to risk stratification in the same level of hypertension group showed that BNP level increased with the risk stratification, the difference was statistically significant (P <0.05) . Conclusion Plasma BNP levels in patients with essential hypertension are significantly higher than those in healthy controls. BNP levels are closely related to blood pressure classification, positively correlated with the degree of left ventricular hypertrophy, and increased with the rise of risk stratification. BNP may become an index of risk stratification of essential hypertension.
