目的探析淋巴细胞CD28、CD86、CD80共刺激分子阳性表达及白介素18(IL-18)在特发性血小板减少性紫癜(ITP)发病过程中的作用。方法分析45例ITP患儿的临床资料,选取同期接受健康体检的45例健康儿童作为对照组,比较两组儿童的基线资料、淋巴细胞CD28、CD86、CD80阳性表达率及IL-18浓度,相关性分析CD86+、CD80+及IL-18与血小板水平间的关系。结果两组儿童基线资料差异无统计学意义(P>0.05)。ITP组的CD86+、CD80+百分比以及IL-18水平明显高于对照组,差异有统计学意义(P<0.01),而两组的CD28+百分值差异无统计学意义(P>0.05)。CD86+及CD80+与血小板水平相关性不明显(P>0.05),而IL-18含量与血小板水平呈明显负相关性(P<0.05)。结论 ITP患儿外周血淋巴细胞CD86、CD80表达过度,其血浆IL-18影响了ITP的发病过程。
Objective To investigate the positive expression of costimulatory molecules CD28, CD86 and CD80 on lymphocytes and the role of interleukin 18 (IL-18) in the pathogenesis of idiopathic thrombocytopenic purpura (ITP). Methods Forty-five children with ITP were enrolled in this study. Forty five healthy children undergoing physical examination at the same period were selected as control group. The baseline data of two groups of children were compared. The positive rates of CD28, CD86, CD80 and IL-18 Sex analysis of the relationship between CD86 +, CD80 +, and IL-18 and platelet levels. Results There was no significant difference in baseline data between the two groups (P> 0.05). The percentages of CD86 +, CD80 + and IL-18 in ITP group were significantly higher than those in control group (P <0.01). There was no significant difference in percentage of CD28 + between the two groups (P> 0.05). The correlation between CD86 + and CD80 + and platelet level was not significant (P> 0.05), while the level of IL-18 was negatively correlated with platelet level (P <0.05). Conclusion The expression of CD86 and CD80 in peripheral blood lymphocytes of children with ITP is excessive, and the plasma IL-18 affects the pathogenesis of ITP.
