目的分析Graves眼病患者骨代谢状况。方法比较60例Graves眼病患者(A组)及52例健康人(B组)血清骨代谢指标,并经双能X射线骨密度仪测定两组L1~L4、左股骨颈及左髋骨密度。结果 A组血清Ⅰ型原胶原N-端前肽、血清β胶原特殊序列和血清ALP高于B组(P<0.05),而25羟维生素D水平低于B组(P<0.05)。A组左股骨颈和左髋的骨密度值低于B组(P<0.05),而两组患者L1~L4的骨密度值差异无统计学意义(P>0.05)。结论 Graves眼病患者骨转换加速,股骨颈和髋部骨密度明显低于正常人;免疫抑制治疗有可能加剧骨代谢的恶化。
Objective To analyze the status of bone metabolism in patients with Graves ophthalmopathy. Methods Sixty patients with Graves ophthalmopathy (group A) and 52 healthy subjects (group B) were enrolled in the study. The bone mineral density (BMD) of L1 ~ L4, left femoral neck and left hip were measured by dual energy X-ray absorptiometry. Results Serum type Ⅰ procollagen N-terminal propeptide, special sequence of serum β-collagen and serum ALP in group A were higher than those in group B (P <0.05), while serum levels of 25-hydroxyvitamin D were lower than those in group B (P <0.05). The BMD of left femoral neck and left hip in group A was lower than that in group B (P <0.05). There was no significant difference in BMD of L1 ~ L4 between the two groups (P> 0.05). Conclusions The bone turnover of Graves ophthalmopathy is accelerated, and the BMD of femoral neck and hip is obviously lower than that of normal people. Immunosuppressive therapy may aggravate the deterioration of bone metabolism.
