目的探讨表观弥散系数(apparent diffusion coefficient,ADC)与非小细胞肺癌(non-small cell lung cancer,NSCLC)常见的预后分子生物学指标的相关性。方法 39例经病理证实为NSCLC的患者接受磁共振弥散加权成像(diffusion weighted imaging,DWI)检查,测量病变的ADC值。采用单因素方差分析及t检验分析不同生物学指标如血中癌胚抗原(cancer embryo antigen,CEA)、组织中Ki67、甲状腺转录因子1(thyroid transcription factor 1,TTF-1)、表皮生长因子受体(epidermal growth factor receptor,EGFR)之间ADC值的差异,采用Spearman相关系数分析ADC值与不同生物学指标的相关性。结果不同生物学指标CEA、Ki67、TTF-1、EGFR之间的ADC值差异均有统计学意义(P<0.01),且ADC值与上述预后相关的生物学指标均存在负相关(CEA:r=-0.77,P<0.001,Ki67:r=-0.58,P<0.001,TTF-1:r=-0.47,P=0.003,EGFR:r=-0.40,P=0.012)。结论 ADC值与NSCLC常见的预后分子生物学指标CEA、Ki67、TTF-1、EGFR存在负相关关系,ADC值较低往往提示CEA与Ki67水平较高,表明TTF-1阳性及EGFR突变可能。
Objective To investigate the relationship between apparent diffusion coefficient (ADC) and common prognostic molecular markers of non-small cell lung cancer (NSCLC). Methods Thirty-nine patients with NSCLC confirmed by pathology were examined by diffusion weighted imaging (DWI). The ADC value of the lesion was measured. One-way analysis of variance (ANOVA) and t-test were used to analyze the different biological indicators such as CEA, Ki67, TTF-1, The differences of ADC values between epidermal growth factor receptor (EGFR) and Spearman correlation coefficient were used to analyze the correlation between ADC value and different biological indexes. Results There were significant differences in ADC values between CEA, Ki67, TTF-1 and EGFR (P <0.01), and there was a negative correlation between ADC value and the above prognostic biological indicators (CEA: r = -0.77, P <0.001, Ki67: r = -0.58, P <0.001, TTF-1: r = -0.47, P = 0.003, EGFR: r = -0.40, P = 0.012). Conclusions The ADC value is negatively correlated with CEA, Ki67, TTF-1 and EGFR in common NSCLC patients. Low ADC values often indicate a high level of CEA and Ki67, indicating that TTF-1 positive and EGFR mutation may be possible.
