目的探讨螺旋(MSCT)增强扫描造影剂注射速度对小肝癌诊断的影响ROC分析。方法收集2010年7月至2013年7月我院收治的患有肝硬化病史和慢性肝炎病史、行MSCT多期增强扫描的小肝癌患者60例(76个病灶),随机将所有患者分为两组,分别是3 ml/s组(30例,35个病灶)和5 ml/s组(30例,41个病灶)。对所有患者的MSCT图像由三位临床经验丰富的放射科主治医师作为观察者来进行随机阅片。结果两组患者的平均Az值、灵敏度也没有较为明显的差异(P>0.05),但两组患者之间的阳性预测值、特异度差异有统计学意义(P<0.05)。结论中等注射速度对小肝癌诊断的效果更佳。
Objective To investigate the influence of helical CT (MSCT) enhanced contrast agent injection on the diagnosis of small hepatocellular carcinoma (ROC). METHODS: 60 patients (76 lesions) of small hepatocellular carcinoma who underwent MSCT multi-phase enhanced scans were collected from July 2010 to July 2013 in our hospital with a history of cirrhosis and chronic hepatitis, and all patients were randomly divided into two groups. The groups were 3 ml/s group (30 cases, 35 lesions) and 5 ml/s group (30 cases, 41 lesions). The MSCT images for all patients were randomly read by three clinically experienced radiology attending physicians as observers. Results There was no significant difference in average Az value and sensitivity between the two groups (P>0.05), but there was a statistically significant difference between the positive predictive value and the specificity between the two groups (P<0.05). Conclusion Moderate injection speed is better for the diagnosis of small hepatocellular carcinoma.
