[目的]分析延边农村地区朝鲜族和汉族居民胰岛素抵抗(IR)与代谢综合征(MS)及其组分的相关性.[方法]采用整群抽样和简单随机抽取的方法抽取7 836名延边朝鲜族自治州农村地区常住朝鲜族和汉族进行现况调查.采用家访的方法收集研究对象的一般情况、心脑血管疾病的既往史及家族史等相关信息.通过健康体检准确测量身高、体质量、腰围、臀围、血压等体格指标.取清晨空腹(禁食12h)静脉血,测量胰岛素、血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、血糖等生物化学指标.[结果]不同性别汉族IR组的整体型肥胖、腹型肥胖、高TG血症、低HDL-C血症、高血压、高血糖及MS患病风险高于非IR组,其差异均有统计学意义(P<0.01);不同性别朝鲜族IR组的上述诸多代谢异常(低HDL-C血症除外)的患病风险高于非IR组,其差异均有统计学意义(P<0.01).偏相关分析结果显示,IR与整体型肥胖、腹型肥胖、高TG血症、低HDL-C血症、高血压、高血糖及MS均明显呈正相关,相关系数分别为0.161,0.137,0.125,0.120,0.074,0.422,0.196(P<0.01).[结论]延边农村地区不同民族IR组的整体型肥胖、腹型肥胖、高TG血症、低HDL-C血症、高血压、高血糖及MS患病风险高于非IR组.
[Objective] To analyze the correlation between insulin resistance (IR) and metabolic syndrome (MS) and its components among Korean and Han residents in Yanbian rural areas. [Methods] A total of 7 836 Yanbian Korean and Han nationality in rural areas of Korean Autonomous Prefecture were surveyed by questionnaire.Patients interviewed were used to collect the general information of subjects, past history of cardio-cerebrovascular diseases and family history and other related information.Through physical examination to accurately measure height, body weight, (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), blood glucose [Results] The overall risk of obesity, abdominal obesity, hypercholesteremia, hypo-HDL-C, hypertension, hyperglycemia and MS in IR group were higher than those in non-IR group, (P <0.01). The prevalence of the above mentioned metabolic abnormalities (except for low HDL-C) in Korean ethnic IR patients of different sexes was higher than that of non-IR patients, and the differences were statistically significant (P <0.01), partial correlation analysis showed significant results , IR was positively correlated with total obesity, abdominal obesity, hypertriglyceridemia, low HDL-C, hypertension, hyperglycemia and MS, the correlation coefficients were 0.161,0.137,0.125,0.120,0.074,0.422 , 0.196 (P0.01). [Conclusion] The overall risk of obesity, abdominal obesity, hypercholesteremia, hypo HDL-C, hypertension, hyperglycemia and MS in different ethnic IR groups in Yanbian rural areas is high In non-IR group.
