目的通过测定血清基质金属蛋白酶(MMP)-9、脂联素(APN)水平,分析其与冠状动脉病变程度之间的关系,探讨MMP-9、APN在冠状动脉粥样硬化发生发展中的可能机制及临床意义。方法将确诊冠心病并接受冠状动脉造影的126例患者分为急性冠状动脉综合征(ACS)组62例、稳定型心绞痛(SA)组64例,并选取健康对照64名,采用酶联免疫吸附法(ELISA)测定3组MMP-9、APN水平,比较两者之间的相关性及其与冠状动脉病变程度的相关性。结果 ACS组、SA组的MMP-9水平均高于健康对照组(P<0.05),ACS组的MMP-9水平又高于SA组[分别为(116.5±0.24)ng/mL,(62.23±0.93)ng/mL,P<0.05];ACS组、SA组的APN水平均低于健康对照组(P<0.05),ACS组的APN水平又低于SA组[分别为(6.65±2.24)mg/L,(8.32±2.93)mg/L,P<0.05];冠状动脉病变支数多者MMP-9水平高于冠状动脉病变支数少者,而APN则显著降低;MMP-9与APN呈负相关(r=-0.552,P<0.05);MMP-9水平与Gensini积分呈正相关(r=0.406,P<0.05),APN含量与Gensini积分呈负相关(r=-0.364,P<0.05)。结论冠心病患者MMP-9、APN之间关系密切,并且MMP-9、APN与冠状动脉病变程度密切相关。
OBJECTIVE: To investigate the relationship between MMP-9 and APN in the development of coronary atherosclerosis by measuring the levels of serum MMP-9, APN and the degree of coronary lesions. Mechanism and clinical significance. Methods One hundred and sixty-six patients diagnosed with coronary artery disease undergoing coronary angiography were divided into two groups: acute coronary syndrome (ACS) group (n = 62) and stable angina pectoris group (n = 64). Sixty-four healthy controls were enrolled in this study. Enzyme-linked immunosorbent assay The levels of MMP-9 and APN in three groups were determined by ELISA. The correlation between them and the degree of coronary artery lesion were compared. Results The levels of MMP-9 in ACS group and SA group were significantly higher than those in healthy control group (P <0.05). The levels of MMP-9 in ACS group were significantly higher than those in SA group [(116.5 ± 0.24) ng / mL and 0.93) ng / mL, P <0.05]. The levels of APN in ACS group and SA group were lower than those in healthy control group (P <0.05), and those in ACS group were lower than those in SA group [6.65 ± 2.24 /L,(8.32 ± 2.93) mg / L, P <0.05]. The levels of MMP-9 in patients with coronary artery lesions were significantly higher than those with fewer coronary lesions, while APN was significantly decreased. (R = -0.552, P <0.05). The level of MMP-9 was positively correlated with Gensini score (r = 0.406, P <0.05). The APN content was negatively correlated with Gensini score . Conclusion There is a close relationship between MMP-9 and APN in patients with coronary heart disease, and MMP-9 and APN are closely related to the severity of coronary artery disease.
