目的 探讨既往心绞痛对急性心肌梗死 (AMI)患者冠状动脉侧支循环及左心室功能的影响。方法 行选择性冠状动脉造影和左心室造影的 30 7例首次急性心肌梗死患者 ,分析既往心绞痛与肌酸激酶 (CK)峰值浓度、冠状动脉侧支循环和左心室功能等的关系。结果 ① 30 7例患者中 ,有既往心绞痛者 192例 (A组 ) ,无既往心绞痛者 115例 (B组 )。②A组CK和CK MB峰值浓度均显著低于B组(P <0 0 5 )。③A组梗死相关动脉有良好侧支循环的比率显著高于B组 (P <0 0 5 )。④A组左心室射血分数 (LVEF)显著高于B组 (P <0 0 1) ,而室壁运动Cortina计分明显低于B组(P <0 0 1)。结论 既往心绞痛可能促进冠状动脉侧支循环的建立 ,对急性心肌梗死后的心脏功能起保护作用
Objective To investigate the effect of previous angina pectoris on coronary collateral circulation and left ventricular function in patients with acute myocardial infarction (AMI). Methods A total of 307 patients with first acute myocardial infarction undergoing selective coronary angiography and left ventricular angiography were analyzed for the relationship between previous angina pectoris and creatine kinase (CK) peak concentration, coronary collateral circulation and left ventricular function. Results ① Among the 307 patients, there were 192 patients with previous angina (group A) and 115 patients without previous angina (group B). ② The peak concentrations of CK and CK MB in group A were significantly lower than those in group B (P <0.05). ③ The ratio of infarction-related arteries in group A was significantly higher than that in group B (P <0.05). ④ The left ventricular ejection fraction (LVEF) of group A was significantly higher than that of group B (P <0.01), while the score of wall motion Cortina was significantly lower than that of group B (P <0.01). Conclusion Previous angina pectoris may promote the establishment of coronary collateral circulation and play a protective role on the cardiac function after acute myocardial infarction
