目的:探讨经胸及经食管超声心动图在多发性房间隔缺损(ASD)封堵中的应用价值。方法:9例经超声心动图诊断为2处ASD的患者(7例行经食管超声心动图检查)在经胸超声心动图监测下行经心导管房间隔缺损封堵术。结果:7例行经食管超声心动图检查者,5例明确为2处ASD,1例为单个较大缺损,1例为3处缺损。余2例介入封堵术中造影检查示1例为2处缺损,1例为单个缺损。所有患者均在经胸超声心动图监测下行经心导管ASD封堵获得成功。4例患者用了2个封堵器,5例用了1个封堵器。结论:对于多发性ASD,可应用经胸或经食管超声心动图明确缺损的数量、大小、间距以及边缘距离,然后根据缺损间距选择单个或多个封堵器。
Objective: To investigate the value of transthoracic and transesophageal echocardiography in the occlusion of multiple atrial septal defect (ASD). Methods: Nine patients with ASD diagnosed by two echocardiography (7 transesophageal echocardiography) underwent transcatheter atrial septal defect occlusion under transthoracic echocardiography. Results: Seven patients underwent transesophageal echocardiography. Five patients were identified as two ASDs. One patient had a single large defect and one patient had three defects. The other two cases involved in the occlusion of angiography showed 1 case of 2 defects, 1 case of a single defect. All patients underwent transcatheter ASD with transthoracic echocardiography. Four patients had two occluders and five had one occluder. CONCLUSIONS: For multiple ASD, the number, size, spacing, and marginal distance of the defect can be determined using transthoracic or transesophageal echocardiography and then the single or multiple occluders are selected based on the defect spacing.
