目的探讨普通DR平片与多层螺旋CT三维重建、多平面重建及MRI成像诊断在骨关节骨折的临床观察。方法选择性分析本院2012-2017年接诊的45例普通DR平片检查后发现的可疑骨折或隐匿性骨折的患者行螺旋CT扫描并进行VR重建及多平面重建,部分患者还进行了MRI扫描,综合分析普通DR平片与多层螺旋CT后重建技术及MRI成像显示隐匿性骨折的能力及特长。结果 45例DR平片显示可疑骨折的患者,后来均经过CT确诊为隐匿性骨折,其中3例膝关节外伤的患者因怀疑有半月板损伤还进行了MRI扫描后经证实有胫骨平台骨折及半月板、前交叉韧带撕裂。结论对于隐匿性骨折,多层螺旋CT及后重建技术比普通DR平片诊断更加明确,尤其多平面重建技术对于骨折线的移位情况显示的非常直观,MRI成像对显示骨折后的骨髓水肿及关节韧带的损伤有明显优势,但对骨皮质的显示能力没有CT效果好。
Objective To investigate the clinical observation of common DR plain film and multi-slice spiral CT three-dimensional reconstruction, multiplanar reconstruction and MRI imaging in the diagnosis of fracture of bone and joint. Methods Selective analysis of 45 cases of suspicious or occult fractures found in our hospital from 2012 to 2017 after selective DR examination was performed spiral CT scan and VR reconstruction and multiplanar reconstruction. Some patients also had MRI Scanning, comprehensive analysis of ordinary DR plain film and multi-slice spiral CT reconstruction techniques and MRI imaging showed occult fracture capacity and expertise. Results 45 cases of DR plain film showed suspicious fractures, and later were confirmed by CT as occult fractures, of which 3 cases of knee trauma patients suspected of having meniscus injury was also MRI scan confirmed tibial plateau fractures and half Plate, anterior cruciate ligament tear. Conclusions For occult fractures, MSCT and post-reconstruction techniques are more specific than ordinary DR plain imaging. In particular, the multiplanar reconstruction technique is very intuitive for the displacement of the fracture line. MRI imaging shows bone marrow edema after fracture and Joint ligament injury has obvious advantages, but the display of cortical bone CT no good effect.
