目的探讨乳腺癌微钙化X线特征与其分子分型间的相关性。方法回顾性分析2013年1月至2016年10月陕西省人民医院149例术前乳腺钼靶X线检查发现微钙化并经手术后病理证实为钙化性乳腺癌患者的钼靶X线影像及病理资料,分析微钙化的X线特征与免疫组化分子分型间的相关性。结果单纯钙化在乳腺癌不同分子分型间的分布有统计学差异(χ~2=9.113,P<0.05),钙化主要见于Luminal A型及HER-2过表达型。乳腺癌病灶BI-RADS分类及微钙化X线形态在乳腺癌不同分子分型间的分布均没有统计学差异(χ~2值分别为0.684、2.921、4.282、0.466、3.233、1.004、0.427、0.406,均P>0.05)。簇状和线样及节段样分布的微钙化在乳腺癌不同分子分型间均有显著统计学差异(χ~2值分别为14.919、19.024,均P<0.01),簇状钙化最常见于HER-2过表达型,线样及节段样钙化最常见于Luminal A型。微钙化灶范围在乳腺癌不同分子分型间有统计学差异(χ~2=9.210,P<0.05),范围≥2cm的微钙化灶最常见于HER-2过表达型。结论乳腺癌微钙化X线特征与分子分型间有一定的相关性,能为判断肿瘤生物学行为及患者预后提供一定的参考价值。
Objective To investigate the correlation between the characteristics of breast cancer microcalcifications and its molecular typing. Methods A retrospective analysis of mammography and pathology in 149 patients with calcified breast cancer who were found to have microcalcifications by surgical mastectomy mammography from January 2013 to October 2016 in Shaanxi Provincial People’s Hospital Data to analyze the correlation between the features of microcalcifications and the molecular typing of immunohistochemistry. Results There was a statistically significant difference in the distribution of different calcifications among breast cancer (χ ~ 2 = 9.113, P <0.05). The calcifications were mainly found in Luminal A and HER-2 overexpression groups. Breast cancer lesions BI-RADS classification and micro-calcification X-ray patterns in the distribution of breast cancer different molecular types were not statistically different (χ ~ 2 values were 0.684,2.921,4.282,0.466,3.233,1.004,0.427,0.406 , All P> 0.05). Clustered and linear and segmental microcalcifications showed statistically significant differences among different molecular types of breast cancer (χ ~ 2 = 14.919 and 19.024, respectively, P <0.01). Clustered calcification was most common in HER-2 overexpression, lineage and segmental calcification are most commonly seen in the Luminal A type. The range of microcalcifications was statistically different between different molecular types of breast cancer (χ ~ 2 = 9.210, P <0.05). The most common type of microcalcification in the range of ≥2cm was HER-2 overexpression. Conclusion There is a certain correlation between the characteristics of X-ray and molecular typing of breast cancer, which can provide some reference value for judging the biological behavior of tumor and the prognosis of patients.
