目的研究急性百草枯中毒(acute paraquat poisoning,APP)患者的肝、肾功能各项指标水平变化的临床意义,以探讨百草枯中毒患者肝、肾脏损伤的发病机制。方法 APP患者49例(A组),根据患者服毒量和临床体征,分为轻度(A1组)、中度(A2组)和重度(A3组)中毒组,48h内检测各项肝、肾功能指标。另随机选择49例健康体检者为对照组(B组)。结果 A组与B组比较:肝功能指标(ALT、AST、TP、ALB、TBIL、DBIL)及肾功能指标(BUN、Cr、UA)两组间的差异均有统计学意义(P<0.05)。A1组与B组比较:除TBIL、DBIL两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A2组、A3组与B组比较:肝功能指标(ALT、AST、TP、ALB、TBIL、DBIL)及肾功能指标(BUN、Cr、UA)两组间的差异均有统计学意义(P<0.05)。A1组与A2组比较:除UA两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A1组与A3组比较:除UA两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A2组与A3组比较:除ALT两组间差异有统计学意义外(P<0.05),其余指标间差异均无统计学意义(P>0.05)。结论百草枯中毒患者的肝、肾可有不同程度的损伤,并随中毒程度的加重而加重。
Objective To study the clinical significance of various indexes of liver and kidney function in patients with acute paraquat poisoning (APP) to explore the pathogenesis of liver and kidney injury in paraquat poisoning patients. Methods A total of 49 APP patients (group A) were divided into mild (group A1), moderate (group A2) and severe (group A3) poisoning patients according to the dose and clinical signs of the patients. Functional indicators. Another 49 randomly selected healthy subjects as control group (B group). Results Compared with group B, the difference of liver function indexes (ALT, AST, TP, ALB, TBIL, DBIL) and renal function indexes (BUN, Cr and UA) were statistically significant . There was no significant difference between group A1 and group B except TBIL and DBIL (P> 0.05), and the other indexes were statistically significant (P <0.05); Group A2, Group A3 and Group B: The differences of functional indexes (ALT, AST, TP, ALB, TBIL, DBIL) and renal function indexes (BUN, Cr, UA) were statistically significant (P <0.05). A1 group and A2 group: except UA, there was no significant difference between the two groups (P> 0.05), the other indexes were statistically significant (P <0.05); A1 group and A3 group: except UA difference between the two groups (P <0.05). There was significant difference between the two groups (P <0.05). There was no significant difference between the two groups (P> 0.05) There was no significant difference between the two groups (P> 0.05). Conclusion The paraquat poisoning patients liver and kidney can have different degrees of injury, and aggravated with the aggravation of poisoning.
