目的:通过对非超重和超重肥胖的正常人、糖尿病血缘亲属、IGT及2型糖尿病人空腹的胰岛素(Ⅰ)、C肽(C)和前胰岛素(PI)测定,以了解2型糖尿病人的基础β细胞功能改变。方法:利用RIA。结果:各超重肥胖组(Ⅱ、Ⅳ、Ⅵ、Ⅷ)的I、C和PI水平均高于相应非超重组(Ⅰ、Ⅲ、Ⅴ、Ⅶ)(P<0.05)。无论超重肥胖抑非超重组,即正常人→糖尿病亲属→IGT→糖尿病组的PI水平有逐渐上升趋势(P<0.01)。糖尿病两组(Ⅶ、Ⅷ)的I和C无继续上升,而PI仍继续上升(P<0.01)。糖尿病两组的PI/I(%)比值高于正常人(Ⅰ、Ⅱ)组(P<0.05)。结论:超重肥胖对胰岛β细胞功能确有影响,胰岛β细胞功能改变在IGT阶段已非常明显,在糖尿病阶级更加突出,并出现“不成比例”的PI升高现象。
OBJECTIVE: To investigate the fasting insulin (I), C peptide (C) and proinsulin (PI) in non-overweight and overweight obese healthy people, relatives of blood relatives, IGT and type 2 diabetes patients Basal β-cell function changes. Methods: Using RIA. Results: The levels of I, C and PI in overweight and obesity group (Ⅱ, Ⅳ, Ⅵ and Ⅷ) were higher than those in the corresponding non - overweight group (Ⅰ, Ⅲ, Ⅴ and Ⅶ) (P <0.05). No matter overweight or obese, the level of PI in non-overweight group (normal group, diabetic relatives, IGT group and diabetic group) increased gradually (P <0.01). I and C in the two groups (Ⅶ, Ⅷ) of diabetes did not continue to rise, but PI continued to rise (P <0.01). The ratio of PI / I (%) in diabetes was higher than that in normal people (Ⅰ, Ⅱ) (P <0.05). CONCLUSIONS: Overweight and obesity has a definite effect on pancreatic β-cell function. The changes of pancreatic β-cell function are very obvious during the IGT stage and more prominent in the diabetic stage with “disproportionate” PI increase.
