目的调查住院新生儿维生素D营养状况,分析相关影响因素,指导孕母及新生儿维生素D的合理补充。方法以该院2014年1-12月2 199例住院新生儿为研究对象,在家属知情同意的情况下,采集患儿静脉血2mL,采用同位素稀释超高效液相色谱串联质谱法对25(OH)D水平进行检测并对对影响因素进行分析。结果最终共送检标本2 108例,送检率95.9%。参与调查的住院新生儿25(OH)D水平(14.6±6.6)ng/mL,异常2 036例,占96.6%,其中维生素D缺乏的新生儿1 689例,维生素D不足的新生儿347例。在不同胎龄、胎数、小于胎龄儿、适于胎龄儿等方面,维生素D水平差异无统计学意义(P>0.05)。冬春季出生患儿维生素D水平明显低于夏秋季出生者,差异有统计学意义(P<0.05)。96.6%的患儿维生素D水平明显降低,其中产前母体有异常情况的占61.0%。结论该院住院新生儿维生素D普遍低于正常,冬春季出生的住院新生儿较易表现出维生素D缺乏的情况,新生儿维生素D水平与母体情况密切相关。
Objective To investigate the nutritional status of vitamin D in newborns in hospital and analyze the related factors to guide the rational supplementation of vitamin D in pregnant women and newborns. Methods From January to December 2014, 2,199 newborns in hospital were enrolled in this study. 2 mL of venous blood was collected with the informed consent of their relatives. Isotope dilution-coupled with high performance liquid chromatography-tandem mass spectrometry (LC-MS / MS) ) D levels were tested and the influencing factors were analyzed. As a result, a total of 2 108 specimens were sent out, with a delivery rate of 95.9%. The 25 (OH) D level of in-hospital newborns was (14.6 ± 6.6) ng / mL, with an abnormality of 2 036 cases, accounting for 96.6% of the total, including 1 689 newborns with vitamin D deficiency and 347 newborns with vitamin D deficiency. In different gestational age, number of births, less than gestational age, suitable for gestational age and other aspects, the level of vitamin D was no significant difference (P> 0.05). Vitamin D levels in children born in winter and spring were significantly lower than those born in summer and autumn, the difference was statistically significant (P <0.05). 96.6% of children with vitamin D levels were significantly lower, of which antepartum maternal abnormalities accounted for 61.0%. Conclusion The hospitalized neonates with vitamin D in the hospital are generally lower than normal, and hospitalized newborns born in winter and spring are more likely to show vitamin D deficiency. The level of vitamin D in newborns is closely related to maternal condition.
