1例47岁男性患者,于胆囊切除术后40余天静脉注射六氟化硫微泡59mg+0.9%氯化钠注射液5ml作腹部超声造影检查。在注射过程中患者突然出现胸闷、头晕、恶心、面色苍白、冷汗、呼吸困难及短暂的神志不清。查体:T35.8℃,P100次/min,R25次/min,BP40/0mmHg,全身皮肤潮红,结膜充血,眼睑水肿。心电图示窦性心动过速。血钾2.63mmol/L,钠142mmol/L,氯103mmol/L,诊断为过敏性休克。立即给予患者吸氧、静脉给予地塞米松、肾上腺素、多巴胺、间羟胺、氯化钾和碳酸氢钠等治疗。之后,患者血压及电解质恢复正常,症状逐渐消失。
A 47-year-old male patient underwent intravenous contrast-enhanced ultrasound (SFUS) with sulfur-dioxide microbubble 59 mg + 0.9% sodium chloride 5 ml intravenously 40 days after cholecystectomy. During the injection, the patient suddenly had chest tightness, dizziness, nausea, paleness, cold sweat, difficulty breathing, and brief unconsciousness. Examination: T35.8 ℃, P100 times / min, R25 times / min, BP40 / 0mmHg, systemic skin flushing, conjunctival hyperemia, eyelid edema. Electrocardiogram shows sinus tachycardia. Serum potassium 2.63mmol / L, sodium 142mmol / L, chlorine 103mmol / L, diagnosed as anaphylactic shock. Patients were given oxygen immediately and given intravenously to dexamethasone, epinephrine, dopamine, meta-hydroxylamine, potassium chloride and sodium bicarbonate. After the patient’s blood pressure and electrolytes returned to normal, the symptoms gradually disappear.
