目的 探讨乙型肝炎表面抗原(HBsAg)对慢性乙肝(CHB)患者外周血树突状细胞(DC)分泌IL-12水平的影响。方法 采集23例CHB患者和8例正常健康人的抗凝外周静脉血,分离外周血单个核细胞进行体外培养,在重组人白细胞介素4和重组人粒细胞-巨噬细胞集落刺激因子的作用下培养7d使DC增殖,成熟,以加入HBsAg作为研究组,不加入HBsAg作为对照组,继续培养72h,以ELISA法检测培养上清液中IL-12的水平。结果 健康研究组分泌IL-12的水平较对照组有所增加,但两者间的差异无显著性意义(P>0.05);CHB患者对照组DC分泌IL-12的水平显著低于健康对照组和研究组(分别为P<0.01和 P<0.001),加入HBsAg刺激后,患者研究组DC产生IL-12的能力提高,较无HBsAg时显著增加(P<0.05)。结论 CHB患者DC分泌IL-12的水平较低,HBsAg刺激后能显著增加IL-12的产生,这可能与CHB患者DC不成熟、功能低下有关。
Objective To investigate the effect of hepatitis B surface antigen (HBsAg) on IL-12 secretion by peripheral blood dendritic cells (DCs) in patients with chronic hepatitis B (CHB). Methods Peripheral blood mononuclear cells (PBMC) from 23 CHB patients and 8 normal healthy subjects were collected for in vitro culture. The effects of recombinant human interleukin-4 and rhG-CSF on colony stimulating factor DCs were cultured for 7 days to proliferate and mature DCs, HBsAg was added into the study group, HBsAg was not added as a control group, and the culture was continued for 72 hours. The level of IL-12 in the culture supernatants was detected by ELISA. Results The level of IL-12 secreted by health research group was higher than that of control group, but there was no significant difference between them (P> 0.05). The level of IL-12 secreted by DC in control group was significantly lower than that of healthy control group And the study group (P <0.01 and P <0.001, respectively). After HBsAg stimulation, the ability of DCs to produce IL-12 in the study group was significantly higher than that of the HBsAg group (P <0.05). Conclusions The level of IL-12 secreted by DCs in CHB patients is low, and the production of IL-12 can be significantly increased after HBsAg stimulation, which may be related to the immature and dysfunctional DCs in CHB patients.
