目的探讨经颅多普勒(TCD)和磁共振血管成像(MRA)诊断颅内动脉狭窄的可靠性。方法对128例经过全脑血管造影(DSA)证实存在颅内动脉狭窄的患者,同时进行TCD和MRA检查并进行比较。结果 TCD诊断颅内动脉狭窄的符合率为77.2%,另外有2例遗漏(大脑中动脉),还有3例为假阳性(颈内动脉)。MRA诊断颅内动脉狭窄的符合率为83.5%,没有遗漏,还有12为假阳性(颈内动脉7例,椎基底动脉系统5例)。结论 TCD和MRA检查作为便捷的颅内动脉狭窄筛查方法可靠性较高。
Objective To investigate the reliability of transcranial Doppler (TCD) and magnetic resonance angiography (MRA) in the diagnosis of intracranial arterial stenosis. Methods One hundred and twenty-eight patients with intracranial arterial stenosis confirmed by total cerebral angiography (DSA) were examined by TCD and MRA at the same time and compared. Results The coincidence rate of TCD in diagnosis of intracranial arterial stenosis was 77.2%. In addition, 2 cases were missed (middle cerebral artery) and 3 cases were false positive (internal carotid artery). MRA diagnostic accuracy of intracranial artery stenosis was 83.5%, there is no omission, there are 12 false positives (internal carotid artery in 7 cases, vertebrobasilar system in 5 cases). Conclusions TCD and MRA are more reliable as a convenient screening method for intracranial arterial stenosis.
