目的:观察低温等离子射频消融术对真菌性角膜炎的清创治疗效果。方法:前瞻性病例对照研究。收集2017年12月至2019年12月沈阳市第四人民医院真菌性角膜炎71例(71只眼)。患者随机分为药物联合机械清创组及药物联合低温等离子射频清创组,两组患者根据病情再各分为轻、中、重组。机械组与射频组分别应用手术刀片与低温等离子射频消融术进行病灶清创,术后给予相同的药物治疗。术后随访3个月。比较两组治疗效果。结果:机械组与射频组患者致病菌比较差异无统计学意义(n χ2=0.226,n P=0.893)。术后3周轻度患者的治愈率两组间比较差异有统计学意义(n P=0.044)。术后4周中度患者的治愈率两组间比较差异有统计学意义(n P=0.036)。术后6周重度患者的治愈率两组间比较差异无统计学意义(n P=0.332)。治疗前后最佳矫正视力差异有统计学意义(n F=23.088,n P<0.05)。治疗前后眼压差异无统计学意义(n F=0.812,n P=0.470)。治疗前后角膜内皮细胞密度差异无统计学意义(n F=0.053,n P=0.965)。n 结论:低温等离子射频消融术治疗真菌性角膜炎清创安全有效,能够更快地控制感染,缩短病程。“,”Objective:To observe the efficacy of debridement of low-temperature plasma radiofrequency ablation for the treatment of fungal keratitis.Methods:This was a prospective case-control study. A total of 71 eyes of 71 patients with fungal keratitis were collected from Dec. 2017 to Dec. 2019 in Shenyang Municipal Fourth People’s Hospital. The patients were randomly divided into two groups: drug combined with mechanical debridement group and drug combined with low-temperature plasma radiofrequency ablation debridement group. The two groups were further divided into mild, moderate and severe groups based on patient’s condition. Surgical blade and low-temperature plasma radiofrequency ablation were used to debridement the lesions in the mechanical group and the radiofrequency group, respectively. The same medication was given after the operation. The follow-up time was 3 months. The efficacy of the two groups were compared.Results:There was no significant difference in pathogenic bacteria between the mechanical group and the radiofrequency group(n χ2=0.226, n P=0.893). At 3 weeks after the operation, the difference in cure rate of mild patients was significant between the two groups(n P=0.044). The difference in cure rate of moderate patients at 4 weeks after surgery was statistically significant between the two groups(n P=0.036). There was no significant difference in the cure rate between the two groups at 6 weeks after the operation(n P=0.332). Before and after treatment, there was significant difference in the best corrected visual acuity(n F=23.088, n P<0.05). There was no significant difference in intraocular pressure before and after treatment(n F=0.812, n P=0.470). There was no significant difference in corneal endothelial cell density before and after treatment(n F=0.053, n P=0.965).n Conclusion:Low-temperature plasma radiofrequency ablation is safe and effective for the treatment of debridement of fungal keratitis, which can control infection faster and shorten the course of illness.
