目的探讨阴道宫颈分泌物中胎儿纤维连接蛋白(fFN)联合宫颈长度(CL)在双胎妊娠早产预测中的价值。方法 50例先兆早产的双胎妊娠孕妇为研究对象,检测孕妇阴道宫颈分泌物中fFN,B超测量CL,观察fFN、CL分别检测及联合检测预测双胎妊娠早产的敏感度、特异度和阳性预测值,分析其对早产的预测价值。结果 fFN、CL预测早产的敏感度(83.33%VS 85.71%)、特异度(71.05%VS 72.22%)和阳性预测值(47.62%VS 54.55%)比较,差异均无统计学意义(P>0.05)。fFN、CL预测早产的敏感度(83.33%、85.71%)与二者联合检测预测早产的敏感度(84.62%)比较,差异均无统计学意义(P>0.05);f FN、CL预测早产的特异度(71.05%、72.22%)和阳性预测值(47.62%、54.55%)均低于二者联合检测预测早产的特异度(97.30%)和阳性预测值(91.67%),差异均具有统计学意义(P<0.05)。结论双胎妊娠中联合阴道宫颈分泌物fFN和CL测量,增加了其阳性预测值和特异度,对双胎妊娠早产有较好的预测意义,值得推广。
Objective To investigate the value of fetal fibronectin (fFN) combined with cervical length (CL) in vaginal cervical secretions in the prediction of preterm birth in twin pregnancy. Methods Fifty pregnant women with twins who had threatened preterm labor were enrolled in this study. The fFN was measured in vaginal cervical secretions of pregnant women. CL was measured in B ultrasound. The sensitivity, specificity and positive predictive value of fFN, Predict the value, analyze its predictive value of preterm labor. Results There was no significant difference in sensitivity (83.33% VS 85.71%), specificity (71.05% VS 72.22%) and positive predictive value (47.62% VS 54.55%) between preterm delivery and fFN, . fFN and CL had no significant difference between the two groups (83.33%, 85.71%) and the combined detection of preterm labor (84.62%) (P> 0.05) The specificity (71.05%, 72.22%) and the positive predictive value (47.62%, 54.55%) were lower than the combined predictive value (97.30%) and the positive predictive value (91.67% Significance (P <0.05). Conclusions The fVN and CL measurements of vaginal cervical secretions in twin pregnancies increase their positive predictive value and specificity, which is of good predictive value for preterm birth of twin pregnancies and is worthy of promotion.
