目的:分析门诊药房退药原因,促进临床合理用药。方法:将退药原因归纳为9类,运用管理学上的帕累托图对本院门诊药房退药处方进行分析。结果:造成患者退药的前3项因素(ADR、不合理用药、医生笔误或收费错误)为主要因素,患者病情变化、医务人员因素为次要因素,其余为一般因素。结论:应针对存在的问题制定诸如开展临床药学服务、提高医务人员的服务意识和业务学习、提高服务质量等措施,本院药事管理委员会应制定出台相应的奖惩措施条款以降低退药率。
OBJECTIVE: To analyze the reason of withdrawal from outpatient pharmacy and promote the rational use of medicine. Methods: The reasons for withdrawal were summarized into nine categories, using the management of Pareto chart on the outpatient pharmacy prescriptions for analysis. Results: The first three factors (ADR, irrational medication, doctor’s mistake or mistake in charge) caused by patient withdrawal were the main factors. The patient’s condition changed, the medical staff factor was the secondary factor, and the rest was the general factor. Conclusion: Measures such as carrying out clinical pharmacy services, improving service awareness and business learning of medical personnel, and improving service quality should be formulated according to the existing problems. The Board of Pharmacy of the hospital should formulate corresponding provisions on incentive measures to reduce the withdrawal rate.
