DVCT冠状动脉血管成像患者心率及重组时相与图像质量的关系

目的:分析320排容积CT(DVCT)冠状动脉血管成像图像质量与患者心率及重组时相间的关系。方法:选取2012年6月至2015年5月无锡市锡山人民医院收治的120例行DVCT检查的患者进行研究,以10%作为造影间期,并重建R-R期间的时相图像,分析患者使用DVCT造影检查的图片质量,分析心率对收缩期/舒张期及图像质量的影响。结果:心率增加患者图像质量评分也显著升高,且质量评分升高程度与心率增加程度呈线性关系,其线性回归方程为y=0.022x-0.154(P<0.01,t=11.04);随心率增加,患者3支冠状动脉图像质量差异有统计学意义(P<0.05);随着重组时相的变化患者平均造影图像质量评分呈现明显的波浪状改变,在相当于患者心脏舒张后期及慢速射血期的时相时可见明显的两个波谷;4组患者RCA、LCX、LAD冠状动脉位置分布差异具有统计学意义(P<0.05);患者收缩期比例与心率的回归方程为y=0.411x+19.2(P<0.01,t=10.8);最佳收缩期时相与心率回归方程为y=0.26x+5.76(P<0.01,t=8.9);患者最佳舒张期时相与心率回归方程为y=0.26x+57.6(P<0.01,t=8.9)。结论:使用DVCT行冠状动脉血管成像检查时,其可以显著提高传统CT高心率的临界值。此外依照不同心率对患者检查的最佳重建时相进行预测结果,调整曝光时相,可以有效将患者吸收量降低并提高图像质量。 OBJECTIVE: To analyze the relationship between the image quality of 320-slice CT (DVCT) coronary angiography and the patient’s heart rate and time to reconstitution. Methods: From June 2012 to May 2015, 120 patients who underwent DVCT in Wuxi Xishan People’s Hospital were enrolled in this study. 10% of the patients were enrolled in this study. The phase images were reconstructed during RR period. The patients underwent DVCT The quality of the image of the contrast examination and the effect of heart rate on systolic / diastolic and image quality were analyzed. Results: The image quality score of patients with heart rate increased significantly, and the level of quality score increased linearly with the increase of heart rate. The linear regression equation was y = 0.022x-0.154 (P <0.01, t = 11.04) (P <0.05). With the change of the reconstitution phase, the average radiographic image quality scores of the patients showed obvious wave-like changes, and in the period corresponding to the late diastole and slow There were significant differences between the two groups during the ejection phase. The distribution of RCA, LCX and LAD coronary arteries in the 4 groups was statistically significant (P <0.05). The regression equation of systolic phase and heart rate was y = 0.411 x + 19.2 (P <0.01, t = 10.8). The optimal systolic phase and heart rate regression equation was y = 0.26x + 5.76 (P <0.01, t = 8.9) The equation is y = 0.26x + 57.6 (P <0.01, t = 8.9). Conclusion: When using DVCT to perform coronary angiography, it can significantly increase the critical value of traditional CT high heart rate. In addition, according to the different heart rate of the patient to check the optimal reconstruction phase of the forecast results, adjust the exposure phase, the patient can reduce the amount of absorption and improve image quality.
