目的 :建立放、化疗粒子联合植入法综合治疗进展期胰腺癌。方法 :2 6例进展期胰腺癌中胰头癌 19例 ,胰体癌 7例。肿瘤最大直径均大于 4cm ,大于 5cm者 5例。行内引流术 2 2例。在治疗计划指导下 ,于术中先植入 5 氟尿嘧啶 ( 5 FU)缓释化疗粒子 ,再植入放射性1 2 5I粒子 ,肿瘤匹配周边剂量 (matchedperipheraldose ,MPD)为 60~ 80Gy。平均每例使用1 2 5I粒子 16粒和5 FU 10 0 0mg。结果 :全部病例经过 3~ 19个月随访观察 ,无手术并发症发生。 16例瘤体缩小超过四分之一。 2 6例患者中位生存期为 12个月 ,最长 1例随访时间为术后 19个月 ,现仍存活。结论 :放射性1 2 5I粒子和 5 FU缓释化疗粒子联合应用局部植入技术是综合治疗进展期胰腺癌的有效手段之一
Objective: To establish radiotherapy and chemotherapy combined with particle implantation in the treatment of advanced pancreatic cancer. Methods: Twenty-nine patients with pancreatic head cancer in 26 cases of advanced pancreatic cancer and 7 cases of pancreatic body cancer. The largest diameter of tumor were greater than 4cm, more than 5cm in 5 cases. In-line drainage in 22 cases. Under the guidance of the treatment plan, 5 Fluorouracil (5 FU) sustained release chemotherapy particles were implanted intraoperatively, and 125 125 I seeds were reimplanted. The matched peripheral hemiphosphorus (MPD) was 60-80 Gy. An average of 1 251 I particles per particle and 5 FU 100 mg are used. Results: All cases were followed up for 3 to 19 months without complications. 16 cases of tumor shrink more than a quarter. Twenty-six patients had a median survival of 12 months, and a maximum of 1 patient was followed up for 19 months postoperatively and is still alive. CONCLUSIONS: The combination of radioactive 125I particles and 5 FU sustained-release chemotherapy particles in combination with local implantation is an effective method for the comprehensive treatment of advanced pancreatic cancer
