目的 探讨一层缝合法在高位及低位消化道吻合中的应用价值。方法 将一层缝合法应用于食管 -空肠吻合 2 9例 ,食管 -胃吻合 2 5例 ,结肠 -直肠吻合 12 6例 ,并观察其并发症发生率。结果 食管 -空肠吻合组吻合口瘘 1例 ,占 3.4 5 % ;食管 -胃吻合组吻合口瘘 1例 ,占 4 .0 0 % ;结肠 -直肠吻合组吻合口瘘 8例 ,占 6 .35 %。全组无明显吻合口狭窄。结论 一层缝合法应用于暴露困难的高位及低位消化道吻合 ,简化了操作 ,吻合口瘘或狭窄等并发症率较低。同时减少了胃底贲门癌开胸机会 ,提高了低位直肠癌的保肛率。
Objective To explore the application value of one-layer suture method in high and low gastrointestinal anastomosis. Methods One layer suture method was applied to 29 cases of esophageal-jejunal anastomosis, 25 cases of esophageal-gastric anastomosis and 12 6 cases of colorectal anastomosis, and the complication rate was observed. Results One case of esophagogastrojejunostomy was anastomotic fistula, accounting for 3.4 5%. One case of anastomotic fistula was esophageal-gastric anastomosis group, accounting for 4.0%. There were 8 anastomotic fistulas in colon-rectal anastomosis group, accounting for 6.35% %. The whole group no obvious anastomotic stenosis. Conclusions The one-layer suture method is suitable for the anastomosis of high and low digestive tract with difficult exposure, which simplifies the operation. The incidence of complications such as anastomotic fistula or stenosis is low. At the same time reduce the chance of thoracic cardia cancer thoracotomy, improve the low rate of rectal cancer anal sphincter.
