目的:探讨磁共振成像相位对比技术(PC-MRI)评估慢性阻塞性肺疾病(COPD)合并肺动脉高压(PH)患者肺动脉形态的价值。方法:抽取周口市第一人民医院2015年7月至2020年3月就诊的72例COPD合并PH患者为研究对象,所有患者均行光学相干断层扫描技术、右心导管等检查,参照相关标准,将肺动脉形态异常患者纳为肺动脉形态异常组,其他患者纳入肺动脉形态正常组;全部患者均接受PC-MRI检查,比较两组主肺动脉PC-MRI相关检查参数(血流峰值速度、血流速度、血流量、顺应性),并分析主肺动脉PC-MRI相关检查参数预测COPD合并PH患者肺动脉形态异常风险价值。结果:72例COPD合并PH患者,24例肺动脉形态异常,占33.33%;肺动脉形态异常组主肺动脉血流峰值速度、血流速度、血流量、顺应性均低于肺动脉形态正常组,差异有统计学意义(n P0.80,均有一定预测价值,且当四者cut-off值分别取57.095 cm/s、6.363 cm/s、52.208 ml、25.625%时,可获最佳预测价值。n 结论:PC-MRI技术对COPD合并PH患者肺动脉形态有一定评估价值,其相应参数可预测患者肺动脉异常风险,未来可考虑用PC-MRI技术评估COPD合并PH患者肺动脉异常风险。“,”Objective:To investigate the evaluation value of magnetic resonance imaging phase contrast technology (PC-MRI) on pulmonary artery morphology in patients with chronic obstructive pulmonary disease (COPD) and pulmonary hypertension (PH).Methods:Seventy-two patients with COPD and PH from July 2015 to March 2020 treated in the First People’s Hospital of Zhoukou were selected as the research objects, all patients underwent the examinations of optical coherence tomography and right ventricular catheterization, according to the relevant standards, patients with abnormal pulmonary artery morphology were included in the abnormal pulmonary artery group, other patients were included in the normal pulmonary artery group; all patients received PC-MRI examination, PC-MRI related parameters (peak velocity, blood flow velocity, blood flow and compliance) of aortopulmonary artery were compared between the two groups, the risk value of PC-MRI related parameters of the main pulmonary artery in predicting the abnormal pulmonary artery morphology in patients with COPD and PH was analyzed.Results:Among the 72 patients with COPD and PH, 24 cases had abnormal pulmonary artery morphology, accounting for 33.33%; the peak velocity, blood flow velocity, blood flow and compliance of main pulmonary artery in abnormal pulmonary artery group were lower than those in normal pulmonary artery group(n P0.80, which had certain predictive value, when the cut-off values of the 4 methods were 57.095 cm/s, 6.363 cm/s, 52.208 ml and 25.625%, the best predictive value could be obtained.n Conclusions:PC-MRI technology has a certain value in the evaluation of pulmonary artery morphology in patients with COPD and PH, the corresponding parameters can predict the risk of pulmonary artery abnormalities, PC-MRI can be used to assess the risk of pulmonary artery abnormalities in patients with COPD and PH in the future.
