目的:初步探讨18FDG-PET对孤立性肺结节(SPN)的诊断价值。方法:以病理组织学诊断作为金标准,回顾性分析18FDG-PET对SPN的诊断作用及假阳性和假阴性的可能原因。结果:2001年4月至2004年5月共有26例患者进入研究,18FDG-PET的敏感性、特异性、准确性、阳性预测值和阴性预测值分别为87.0%、66.7%、84.5%、95.2%和40.0%。恶性病变组与良性病变组最大标准化摄取值(SUV)的差异没有显著性。结论:18FDG-PET诊断SPN的敏感性高,特异性低。肺结核可能是假阳性的原因;而腺癌和原发癌灶太小等可能是假阴性的原因。
Objective: To investigate the value of 18FDG-PET in the diagnosis of solitary pulmonary nodules (SPN). Methods: With histopathological diagnosis as the gold standard, the diagnostic value of 18FDG-PET for SPN and the possible causes of false-positive and false-negative were retrospectively analyzed. Results: A total of 26 patients were enrolled in the study from April 2001 to May 2004. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18FDG-PET were 87.0%, 66.7%, 84.5%, 95.2 % And 40.0%. There was no significant difference in maximum standardized intake (SUV) between the malignant and benign groups. Conclusion: 18FDG-PET is highly sensitive and specific for the diagnosis of SPN. Tuberculosis may be the cause of false positives; adenocarcinoma and primary cancers are too small may be the cause of false negative.
