背景:艾滋病患者脑皮质和皮质下局部脑血流减少,但无痴呆综合征的艾滋病患者局部脑血流的变化尚不清楚。目的:观察采用99锝m-双半胱氨酸乙酯脑单光子发射型计算机断层显像(singlephotonemissioncomputerizedtomography,SPECT)的无痴呆综合征艾滋病患者局部脑血流的变化情况,以期为无痴呆综合征艾滋病患者发生痴呆进行一级康复预防和早期干预提供参考依据。设计:以无痴呆综合征的艾滋病患者为研究对象,健康人群为对照组的观察对比研究。单位:一所大学医院的核医学科。对象:研究对象为1999-02/1999-07在暨南大学附属第一医院核医学科接受脑SPECT检查的4例无痴呆综合征的中国广东籍男性艾滋病患者,年龄31~36岁,平均34岁。选择1999-02/2000-06在暨南大学附属第一医院接受脑SPECT检查的16例健康男性作为对照组,年龄21~48岁,平均37岁,临床检查未发现神经精神异常。方法:对两组进行99锝m-双半胱氨酸乙酯脑血流灌注显像,使用双探头SPECT采集图像数据。采用半定量分析软件测定局部脑血流,比较两组局部脑血流变化。主要观察指标:两组局部脑血流半定量分析结果比较。结果:4例无痴呆综合征艾滋病患者的双侧额、顶、颞叶、基底节和丘脑、以及直回和桥脑局部脑血流明显低于正常对照组(P<0.01)。结论:应用SPECT观察无
BACKGROUND: Local cerebral blood flow decreases in the cortex and subcortex of AIDS patients, but changes in regional cerebral blood flow in AIDS-free patients without dementia syndrome remain unclear. Objective: To observe the changes of local cerebral blood flow in patients with dementia without dementia syndrome using 99 technetium m-bis-cysteine ethyl ester single photon emission computerized tomography (SPECT) AIDS patients with dementia to provide a reference for prevention and early intervention. Design: AIDS-free patients with dementia-free syndrome as a research object, the healthy population as a control group observed comparative study. Unit: a university hospital nuclear medicine. PARTICIPANTS: A total of 4 Chinese patients with AIDS-free males from Guangdong, aged 31-36 years, with an average of 34 years of age undergoing brain SPECT from the Department of Nuclear Medicine, The First Affiliated Hospital of Jinan University from February 1999 to July 1999, . From January 1999 to June 2000, 16 healthy male patients who underwent brain SPECT in the First Affiliated Hospital of Jinan University were selected as the control group, aged from 21 to 48 years, with an average of 37 years old. No neuropsychiatric abnormalities were found in the clinical examination. METHODS: Cerebral perfusion imaging of 99 technetium m-bis-cysteine ethyl ester was performed on both groups and image data were acquired using dual-probe SPECT. Local cerebral blood flow was measured by semi-quantitative analysis software, and the changes of regional cerebral blood flow were compared between the two groups. MAIN OUTCOME MEASURES: Results of semi-quantitative analysis of regional cerebral blood flow in two groups were compared. Results: The bilateral frontal, parietal and temporal lobe, basal ganglia and thalamus, as well as the regional cerebral blood flow in direct and pontine were significantly lower than those in the normal control (P <0.01).
