目的探索社区0~12月龄儿童发展干预模式,促进早期儿童发展。方法2005-06—2006-08在北京市城镇12个保健科采取病例对照、纵向追踪、随机整群抽样的研究方法,共收集数据210份,试验组66例、对照1组74例、对照2组70例。按各组的工作流程追访1年(6个月以内1个月1次,6~12个月每2个月1次)。采用Gesell发育诊断法、HOME量表、依恋问卷评估干预模式效果。结果试验组的安全依恋行为(F=4.35,P<0.05)、父母提供适宜玩具(F=5.37,P<0.01)、丰富环境刺激(F=5.87,P<0.01)与对照1组、2组比较差异均有显著性,在家居组织环境(均数差=0.49,P<0.01)方面好于对照1组,在语言发育(均数差=5.12,P<0.05)、个人-社会能力发展(均数差=7.57,P<0.05)方面好于对照2组;试验组及对照1组的保健费用低而保健模式满意率高于对照2组。结论综合干预模式能够通过改善父母的养育知识及行为,促进早期儿童发育及社会性的发展;具有发育监测作用,适宜在社区儿童保健中使用。
Objective To explore the development intervention model for children aged 0-12 months in community to promote the development of early childhood. METHODS: A total of 210 cases were collected from 12 health departments in urban areas of Beijing, including case-control, longitudinal follow-up and randomized cluster sampling. Data were collected from 66 patients in trial group and 74 in control group 1, Group of 70 cases. Follow the workflow of each group for 1 year (once within 6 months, once every 2 months from 6 to 12 months). The Gesell developmental diagnosis, the HOME scale and the attachment questionnaire were used to evaluate the effect of the intervention model. Results The safety attachment behavior of the experimental group was significantly higher than that of the control group (F = 4.35, P <0.05). Parents provided the appropriate toys (F = 5.37, P <0.01) Compared with control group 1, there was significant difference in the home environment (mean difference = 0.49, P <0.01). There were significant differences in language development (mean difference = 5.12, P <0.05), personal-social ability development Mean difference = 7.57, P <0.05) was better than the control group 2; the experimental group and control group 1 health care costs are low and the satisfaction rate of health mode is higher than the control group 2. Conclusion The comprehensive intervention model can promote the development of early childhood and social development by improving parents’ parenting knowledge and behavior. It has the function of development monitoring and is suitable for use in community child health care.
