【目的】研究妊娠期高血压疾病(hypertension disorder complicating pregnancy,HDCP)对围产儿结局的影响及胎儿脐动脉血流变化与围产儿不良结局的关系。【方法】对57例妊娠期高血压疾病孕产妇和59例正常妊娠孕产妇的妊娠结局进行观察,测量胎儿脐动脉血流指标并观察两组围产儿结局。【结果】HDCP组围产儿的不良结局在新生儿窒息、早产和低出生体重方面的发生率均高于正常组(P<0.05);两组在脐动脉血流搏动指数(PI)、阻力指数(RI)、收缩压最大血流速度与舒张末期最大血流速度比值(S/D)等三个指标上存在的差异有统计学意义(P<0.05)。胎儿脐动脉血流指标与围产儿出生结局呈负相关关系。【结论】HDCP患者更容易发生围产儿不良结局,胎儿脐动脉血流指标更易出现异常。脐动脉血流指标RI和S/D比值可以用来预测围产儿不良结局的发生。
【Objective】 To investigate the effect of HDCP on perinatal outcome and the relationship between fetal umbilical artery blood flow changes and poor outcome in perinatal infants. 【Methods】 The pregnancy outcomes of 57 pregnant women with gestational hypertension and 59 normal pregnant women were observed. The indexes of umbilical artery blood flow were measured and the outcomes of two groups were observed. 【Results】 The incidence of perinatal adverse outcomes in neonates with HDCP was higher in asphyxia, premature birth and low birth weight than in normal controls (P <0.05). The mean arterial blood flow index (PI), resistance index (RI), systolic pressure maximum blood flow velocity and end diastolic blood flow velocity ratio (S / D) were significantly different (P <0.05). Fetal umbilical artery blood flow index and the outcome of perinatal children was negatively correlated. 【Conclusion】 Patients with HDCP are more likely to have poor outcomes in perinatal and fetal umbilical artery blood flow indicators are more prone to abnormalities. The RI and S / D ratio of umbilical artery blood flow index can be used to predict the occurrence of adverse outcomes in perinatal children.
