目的:观察氦-氧混合气治疗慢性阻塞性肺疾患(COPD)合并高二氧化碳血症的临床疗效。方法:选择17例慢性阻塞性肺疾患合并高二氧化碳血症患者,随机分为实验组(8例,吸入He-O2)和对照组(9例,吸入N2-O2),通过自制重复利用氦气的循环式氦-氧混合气辅助呼吸装置进行呼吸,实验中患者依次吸入FiO2分别为21%、40%、60%、80%的氦-氧(实验组)或空-氧(对照组)混合气,观察肺功能及血气的变化。结果:氦-氧混合气可以降低吸气负荷,促进氧合及体内二氧化碳排除;起效迅速,但后续效应短暂;氦-氧混合气的理想治疗浓度为40%~79%氦。结论:临床初步应用证明吸入氦-氧混合气治疗慢性阻塞性肺疾患合并高二氧化碳血症患者具有良好的疗效,循环式氦-氧混合气辅助呼吸装置可以显著节省氦气的消耗
Objective: To observe the clinical efficacy of helium - oxygen mixture in the treatment of chronic obstructive pulmonary disease (COPD) complicated with hypercapnia. Methods: Seventeen patients with chronic obstructive pulmonary disease complicated with hypercapnia were randomly divided into experimental group (n = 8, inhaled He-O2) and control group (n = 9) (Helium-oxygen (experimental group) or air-oxygen group (control group) were inhaled with 21%, 40%, 60% and 80% Gas, observe the lung function and blood gas changes. Results: The helium-oxygen mixture can reduce the inspiratory load and promote the oxygenation and the removal of carbon dioxide in the body. The rapid onset of action, however, has a short follow-up effect. The ideal concentration of helium-oxygen mixture is 40% -79% helium. Conclusion: The preliminary clinical application shows that inhaled helium-oxygen mixture has good curative effect in patients with chronic obstructive pulmonary disease complicated with hypercapnia, and the helium-oxygen assisted breathing apparatus with circulating helium-oxygen mixture can significantly save the consumption of helium
