目的检测多囊卵巢综合征(PCOS)患者卵巢颗粒细胞中microRNA-200b(miR-200b)的表达,分析其与胰岛素抵抗的关系。方法选取PCOS患者120例(其中胰岛素抵抗78例,非胰岛素抵抗42例)及对照患者72例。采用逆转录定量PCR(qRT-PCR)检测在卵巢颗粒细胞中miR-200b的表达,并分析其临床意义。结果 PCOS胰岛素抵抗患者中超重或肥胖、高血压、高血糖、脂代谢紊乱和代谢综合征的发生率皆高于PCOS非胰岛素抵抗患者和对照患者(P均<0.05)。PCOS胰岛素抵抗患者miR-200b表达上调,与PCOS非胰岛素抵抗患者和对照患者比较差异有统计学意义(P=0.012,P<0.001);且PCOS胰岛素抵抗患者中miR-200b表达量与胰岛素抵抗指数(HOMA-IR)呈正相关(r=0.532,P=0.002)。结论 PCOS胰岛素抵抗患者高血压等代谢方面远期并发症的患病率较高。miR-200b高表达与PCOS胰岛素抵抗呈正相关,可能是PCOS患者出现胰岛素抵抗的重要病理机制之一。
Objective To detect the expression of microRNA-200b (miR-200b) in ovarian granulosa cells from patients with polycystic ovary syndrome (PCOS) and analyze its relationship with insulin resistance. Methods 120 cases of PCOS patients (including insulin resistance in 78 cases, non-insulin resistance in 42 cases) and control patients in 72 cases. The expression of miR-200b in ovarian granulosa cells was detected by reverse transcription-polymerase chain reaction (qRT-PCR) and its clinical significance was analyzed. Results The incidence of overweight or obesity, hypertension, hyperglycemia, lipid metabolism disorder and metabolic syndrome in PCOS patients with insulin resistance were higher than those in PCOS non-insulin resistance patients and controls (all P <0.05). The expression of miR-200b in PCOS patients with insulin resistance was up-regulated, which was significantly different from those in non-insulin resistant PCOS patients and control patients (P = 0.012, P <0.001). The expression of miR-200b and insulin resistance index (HOMA-IR) was positively correlated (r = 0.532, P = 0.002). Conclusions The prevalence of long-term complications such as hypertension and other metabolic complications in PCOS patients with insulin resistance is high. The high expression of miR-200b is positively correlated with insulin resistance in PCOS, which may be one of the important pathological mechanisms of insulin resistance in PCOS patients.
