目的探讨原发性高血压对良性前列腺增生(BPH)组织中血管新生的影响。方法应用免疫组织化学方法检测了50例单纯BPH和50例BPH并发高血压患者的前列腺组织中CD34抗原及血管内皮生长因子(VEGF)的表达,并计算微血管密度(MVD)。结果BPH并发高血压组MVD明显高于单纯BPH组(P<0.01)。BPH并发高血压组内,高血压病程≥15年组MVD明显大于高血压病程<15年组(P<0.05);高血压病程与前列腺容积有明显相关性。BPH并发高血压组VEGF在前列腺间质中的阳性表达率显著高于单纯BPH组(P<0.05)。BPH并发高血压组前列腺组织的MVD与间质中VEGF表达呈显著正相关(r=0.11,P<0.05)。结论高血压与前列腺间质中VEGF的表达升高密切相关。VEGF的表达升高可以诱导血管新生,因此可以促进并发高血压患者BPH的临床进展。
Objective To investigate the effect of essential hypertension on angiogenesis in benign prostatic hyperplasia (BPH). Methods The expression of CD34 antigen and vascular endothelial growth factor (VEGF) in prostatic tissue of 50 cases of pure BPH and 50 cases of BPH complicated with hypertension were detected by immunohistochemistry and the microvessel density (MVD) was calculated. Results The MVD in BPH complicated hypertensive group was significantly higher than that in pure BPH group (P <0.01). In BPH complicated hypertensive group, the MVD of hypertensive group ≥ 15 years was significantly greater than that of hypertension group <15 years (P <0.05). The course of hypertension was significantly correlated with the volume of prostate. The positive rate of VEGF in BPH group was significantly higher than that in BPH group (P <0.05). There was a significant positive correlation between the MVD of prostate tissue and the expression of VEGF in the BPH-treated hypertensive group (r = 0.11, P <0.05). Conclusion Hypertension is closely related to the increased expression of VEGF in the stroma of prostate. The increased expression of VEGF can induce angiogenesis and therefore promote the clinical progress of BPH in patients with hypertension.
