目的探讨血清基质金属蛋白酶-9(MMP-9)、超敏C反应蛋白(hs-CRP)水平及外周血白细胞(WBC)计数与冠心病发生、发展的关系.方法选取冠心病患者105例作为冠心病组,包括稳定性心绞痛(SAP)组27例、不稳定性心绞痛(UAP)组48例和急性心肌梗死(AMI)组30例,测定血清MMP-9、hs-CRP浓度和外周血WBC计数,健康体检者40名为对照组.结果①冠心病各组MMP-9、hs-CRP、WBC较对照组显著增高(P<0.01);②AMI组血清MMP-9、hs-CRP浓度和外周血WBC计数高于其余三组(P<0.05);③UAP组血清MMP-9、hs-CRP浓度和外周血WBC计数高于其余二组(P<0.05);④SAP组血清hs-CRP浓度高于对照组(P0.05);⑤MMP-9、hs-CRP水平及WBC计数与冠心病事件的严重程度呈正相关(r=0.836,P<0.01;r=0.672, P<0.01;r=0.619,P<0.01);⑥冠心病患者血清MMP-9浓度与hs-CRP浓度及外周血WBC计数呈正相关(r=0.559,P<0.01;r=0.553,P<0.01).结论血清MMP-9、hs-CRP与外周血WBC计数与冠心病不同类型及斑块稳定性有关.“,”Objective To investigate whether levels of matrix metalloproteinase-9(MMP-9),high sensitive C-reactive protein(hs-CRP) and white blood cell(WBC) count were affected in patients with coronary heart disease(CHD).Methods The levels of MMP-9,hs-CRP and WBC were measured in 27 patients with stable angina pectoris(SAP),48 patients with unstable angina pectoris(UAP),30 patients with acute myocardial infarction(AMI) and 40 control normal subjects.Results ①The levels of MMP-9,hs-CRP and WBC in AMI,UAP,and SAP groups were significantly higher than those of the control group(P<0.01).②The levels of MMP-9,hs-CRP and WBC in AMI group were markedly higher than UAP,SAP and control groups(P<0.05).③The levels of MMP-9,hs-CRP and WBC in UAP group were significantly higher than SAP and control groups(P<0.05).④The levels of hs-CRP in SAP group were significantly higher than those in control group(P0.05).⑤The levels of MMP-9,hs-CRP and WBC were positively correlated with the severity of coronary events(r=0.836,P<0.01;r=0.672,P<0.01;r=0.619,P<0.01).⑥Significant positive correlation between the levels of MMP-9 and hs-CRP and WBC all were observed in the patients with CHD(P<0.05).Conclusion The elevation of levels of MMP-9,hs-CRP and WBC were associated with initiation and progression of CHD,and they may help predict cardiovascular events.
